Ce sont en effet parmi les plus graves des manifestations d'une iatrogénie médicamenteuse qui est globalement perçue comme un important problème de Santé Publique.
Le risque de SCAR n'est pas détectable dans les phases précliniques du développement et exceptionnellement dans les études cliniques précédant l'AMM. (C’est dans les premières années de commercialisation que le risque apparait, pouvant conduire à un retrait d'AMM, de coût majeur pour l'Industrie du Médicament.
Le groupe Européen RegiSCAR (Allemagne, Autriche, France, Israël, Italie, Pays-Bas, UK) s’est constitué depuis 1989 sous les noms préalables de SCAR puis d'EuroSCAR. Il pratique une détection active des SCAR dans une population d’environ 150 millions de personnes, grâce à un réseau efficace de correspondants hospitaliers. Il a collecté plus de 2000 cas de SCAR valides avec des données cliniques homogènes et détaillées. Il a réalisé deux études cas-témoins évaluant le risque médicamenteux et son évolution dans Ie temps. Ces études ont alerté sur des médicaments non soupçonnés, confirmé d'autres alertes mais aussi « innocenté » de nombreux médicaments incriminés dans des « case reports ». Le suivi prospectif d'une cohorte de plus de 300 malades atteints de SJS/TEN a montré que la gravité initiale et plus encore la fréquence et la gravité des séquelles avaient été sous-estimées.

The above are examples of the most serious cases of adverse drug reaction, which is widely considered to be a major Public Health problem.
It is impossible to detect the risk of SCAR during the pre-clinical phase of development and it is only rarely detected during the clinical phases prior to approval by the relevant regulatory authorities (AMM in France). The risk usually becomes apparent only during the first years of marketing and this can lead to it being pulled off the market at great cost to the Pharmaceutical Industry.
