Source text - English Retinoid research in the field of dermatology has been influenced by the clinical success of topical tretinoin and oral isotretinoin in the treatment of acne, and by the discovery of high-affinity binding proteins for retinoic acid mediating its action and interaction with other vitamins and hormones. Objective: We sought molecules with an optimal balance between stability, efficacy, and local tolerance for topical acne therapy. Methods: In vitro and in vivo bioassay systems were used to test the ability of retinoids to modulate cell proliferation and differentiation. In addition, anti-inflammatory properties were assessed. Binding and transactivation assays were used to compare affinities and transcriptional activities of adapalene and tretinoin for the nuclear transcription factors, retinoic acid receptors (RARs). Results and Conclusion: Adapalene is a stable naphthoic acid derivative with potent retinoid pharmacology, controlling cell proliferation and differentiation. In addition it has significant anti-inflammatory action. The nuclear gene transcription factors RARβ and RARγ mediate the retinoid activity of adapalene.Mechanism of action/Effect:
The exact mechanism is not known. Adapalene exhibits some retinoic acid-like activity but it also has additional effects. It is thought that adapalene reduces important features of the pathology of acne vulgaris by normalizing the differentiation of follicular epithelial cells and keratinization to prevent microcomedone formation, similar to the mechanism of retinoic acid. Unlike retinoic acid, adapalene selectively binds to some nuclear retinoic acid receptors (RARs) and does not bind to cellular receptors called cytosolic retinoic acid binding proteins (CRABPs). Relatively selective affinity for specific nuclear retinoic acid receptor (RAR) proteins (e.g., RARβ, RARγ) that appear to enhance gene transcription.It is hypothesized that by selectively binding to certain nuclear retinoic acid receptors and not others, adapalene enhances keratinocyte differentiation without inducing epidermal hyperplasia and severe irritation, such as is seen with retinoic acid.

Azithromycin is the first class of antibiotics designated chemically as azalide derived by insertion of a nitrogen atom into the lactone ring of erythromycin.This structural modification results in resistance to acid degradation, improved tissue-penetration, improved activity against gram-negative organisms and prolonged elimination half-life. Azithromycin is usually bacteriostatic. although it may be bactericidal in high concentrations, it inhibits protein synthesis in susceptible organisms by binding to the 50S ribosomal subunits. Azithromycin is rapidly absorbed from the Gl tract and distributed into most body tissues and fluids. It is characterized by low plasma concentrations but high and persistent tissue concentrations.
INDICATIONS:
XXXXXXXXXXX 250mg capsules is indicated for respiratory tract infections, otitis media, skin and soft tissue infections and uncomplicated genital chlamydial infections.
DOSAGE AND ADMINISTRATION:
Adults include the elderly patients usually receive 1.5 gm of Xxxxxxxxxxx over 3 days, 500 mg (2 caps) as a single dose per day.
For skin and skin structure infections 500 mg (2 caps) as a single dose in the first day followed by 250 mg one capsule once daily from day 2 through 5.
The same adult dose may be used for children weighing 45 Kg or more.
Sexually transmitted disease due to haemophilus ducreyi (chancroid), chlamydia trachomatis or susceptible neisseria gonorrhoea, the dose is 1000 mg (4 capsules) as a single oral dose.
XXXXXXXXXXX should be given at least 1 hour before or 2 hours after meals.
ADVERSE EVENTS:
Xxxxxxxxxxx is well tolerated with low incidence of adverse events which include :
• Gastrointestinal: Most of the reported adverse events are mild to moderate in severity and reversible upon
discontinuation of the drug, nausea, vomiting, abdominal pain, dyspepsia and diarrhea (rarely resulting dehydration).
• Skin & soft tissue: mild to moderate allergic reactions, photosensitivity, rare severe hypersensitivity have been reported,
• Dizziness, headache and drowsiness may occur.
• Abnormal liver functions including hepatitis or cholestatic jaundice also have been reported.
• Mild neutropenia also has been reported.
CONTRAINDICATIONS:
Azithromycin is contraindicated in patients with hypersensitivity to azithromycin, erythromycin or any macrolide antibiotic and in severe hepatic insufficiency.
PRECAUTIONS:
Patients should be cautioned to take azithromycin capsules at least one hour prior to a meal or at least two hours after a meal.
Patients should also be cautioned not to take aluminium and magnesium containing antacids and azithromycin simultaneously.
The patient should be directed to discontinue azithromycin immediately and contact physician if any sign of allergic reaction occurs.
SPECIAL POPULATION:
There are no adequate or well-controlled studies in pregnant and lactating women, therefore harmful effects to the fetus and child following the intake of azithromycin can not be excluded. Azithromycin should be used in pregnant and lactating women only if the potential benefit justifies the potential risk.
Azithromycin is principally eliminated via the liver, therefore caution should be exercised when azithromycin is adiministered to patients with impaired hepatic function.
WARNING:
Allergic reactions including angioedema, anaphylaxis and dermatologic reactions including Stevens Johnson Syndrome and toxic epidermal necrolysis have been reported in some patients on azithromycin therapy. When symptomatic treatment of allergic reaction discontinued, reoccurrence of allergic reactions have been reported in some patients. If any allergic reaction occurs, the drug should be discontinued and appropriate therapy should be instituted.
Pseudo membranous colitis may occur in patients under treatment with broad spectrum antibiotics, therefore it is important to consider this diagnosis in patients who present with diarrhoea subsequent to the administration of antibacterial agents.
DRUG INTERACTIONS:
Aluminium and magnesium containing antacids reduce the peak serum level of azithromycin, therefore a time interval of 2-3 hours
should be observed.
Macrolide-antibiotics are known to interact with triazolam, cyclosporin and digoxin. For azithromycin, sufficient data are not available
but the possibility of interaction should be borne in mind.

EFFETS INDESIRABLES :
Xxxxxxxxxxx est bien toléré avec une faible incidence d’effets indésirables qui incluent :

CONTRE-INDICATIONS :
L’azithromycine est contre-indiquée chez les patients avec une hypersensibilité à l’azithromycine.
PRECAUTIONS :
Les patients doivent être avertis qu’ils doivent prendre les capsules d’azithromycine au moins 1 heure avant les repas, ou au moins 2 heures après les repas.
Les patients doivent aussi être avertis qu’ils ne doivent pas prendre simultanément des antacides contenant l'aluminium et le magnésium avec l’azithromycine.
Le patient doit arrêter immédiatement le traitement et voir le médecin traitant dans le cas d'apparition de signes de réactions allergiques.
POPULATION SPECIALE :
Il n’y a pas d’études adéquates ou bien contrôlées concernant les femmes enceintes ou allaitant ; par conséquent, des effets nocifs sur le fœtus et l’enfant suite à la prise de l’azithromycine ne peuvent pas être exclus. L’azithromycine doit être utilisée chez la femme enceinte ou allaitant seulement si le bénéfice potentiel justifie le risque potentiel.
L’azithromycine est principalement éliminée par le biais du foie ; par conséquent, des précautions doivent être prises lors de l’administration de l’azithromycine chez des malades avec disfonctionnement hépatique.
MISE EN GARDE :
Des réactions allergiques ont été rapportées chez des patients sous traitement à l’azithromycine, y compris l’œdème de Quincke et l’anaphylaxie, ainsi que des réactions dermatologiques, y compris le syndrome de Stevens Johnson et la nécrolyse épidermique toxique. This allows a "Holter Scan" equivalent report to be prepared for any past 24 hour period that the patient is on the system. Additionally, during the real-time display mode, medical technicians can log on from a remote terminal and display ECG patterns in real time. Furthermore, it has the capability of taking up to 7 days of cardiac data and processing it through specialized scanning software.
