agree  Joseph Tein: I would use 'surname.' My Gran Diccionario Oxford says 'surname' = 'apellido' and I would always use fewer words wherever possible.

disagree  Heidi C: This is a literal translation that makes no sense in US Eng. US Birth Certificates use "mother maiden name". And your example is trying to explain what a 2nd surname is, not using it as a term.

agree  Brian McDougall: Yes, unless the parents have never married, in which case I'd go for "mother's last name", though even that wouldn't work if the mother is married to someone other than the father. By the way. British people wouldn't understand second last name either.

¿La traducción es para fines de información, o para el uso de los no hispanohablantes? Si es el segundo, es muy probable que el usuario de habla inglés no tiene segundo apellido. Entonces, ¿qué le exigirán, que lo deje en blanco o que escriba de todos modos su apellido maternal aunque ése no forma parte de su apellido legal? Como angloparlante (con un solo apellido) viviendo en un país hispanohablante, he topado con ambos casos. Ofrezco esta alternativa para el caso de que quieren el apellido maternal, sea o no parte del nombre oficial de la persona.
