Spanish term or phrase: supuso un duro golpe
La trágica muerte de un joven de 18 años que participaba en uno de estos ensayos clínicos suposo un duro golpe que generó alarma en todo el mundo.

I can´t quite get the right tone here. Tough blow? Or maybe something like "big shock".

Any ideas welcomed.

Many thanks

agree  Tony M: I love the expression, though it's kind of hanging, looking for a "to someone..." that is unfortunately not there // Kinda what I was wondering (context?)

  -> very true. The hard blow need to be dealt to someone or something, maybe the clinical trials, if we turn the sentence around a bit

was a tough blow

Explanation:
Your idea is excellent. As for supuso, another option might be "represented," though I think "was" works well in the context of the entire sentence (i.e., with the verb "generated" that follows).

suposo = represented

Good luck Adam, and regards!

As for "hard" and "blow" together, I'm not so sure this is advisable. That said, George's "dealt" is an excellent option.
