this is basically report form that has to be validated by the appropriate authority.

I was going to suggest "approved installation plan certificate" but "permit to install" is the correct one. Its also used in both the UK and US.

I was going to suggest "approved installation plan certificate" but "permit to install" is the correct one. Its also used in both the UK and US.
