SPAIN. Not sure what to call these "evidencias":

Un cuaderno sencillo sirve para registrar todos los comentarios y anécdotas de los alumnos que indican que han comprendido el libro, que pueden establecer conexiones intertextuales y que logran aprendizajes literarios. Es bueno tenerlo siempre a mano y anotar lo escuchado o sucedido tan pronto los chicos terminan la sesión. "

Explanation:
I was a teacher in another life :) I would leave 'evidencias' out and use 'Journal', or "Teacher's Journal" or 'Progress Journal'. Saludos!

Asker: I prefer to keep "evidence" in reserve for legal contexts. Maybe "observation journal" is the best option...
