CONSIDERANDO
Que, MARÍA con cédula de ciudadanía XXX de Bogotá, ha solicitado autorización del ejercicio profesional como Médica y Cirujana, según título que le otorgó la Universidad del Rosario y Acta de Grado xxxx del 18 de diciembre de 2004.

RESUELVE
ARTÍCULO SEGUNDO: Autorizar a MARÍA, con cédula xxxx, para ejercer la profesión de Médica y Cirujana en todo el Territorio Nacional.

No se trata de la práctica que exigen mientras está en la Universidad, sino de poderse desempeñar profesionalmente en dicho campo y a nivel nacional después de estar graduada.

No sé cómo traducir al inglés "Autorización del ejercicio profesional como..." = Authorization for professional...

or
has applied for a license to practice medicine and perform surgery (this second part actually sounds a little awkward to me, but I don't think we can speak of "practicing surgery" in the sense meant here; that's why this phrase was my second suggestion instead of my first)

The Colombian government does speak of medical licensing (see URL below), and "license to practice" sounds more natively English to me (at least in American English) than "authorization to practice"
