special weighted rate for outpatient chemotherapy
Explanation:
Yes, it refers to national insurance coverage. If a patient gets chemotherapy in an outpatient setting, (s)he will not need admission and the insurer can save a substantial amount. So they set up special rates tp promote this practice. You can use the terms "charge" or "reimbursement" appropriately for the context. See the link below.

outpatient chemotherapy premium

Explanation:
外来化学療法特別加算 -> The term 外来化学療法加算 is generally used.

特別加算 -> 加算 = premium
外来化学療法 = outpatient chemotherapy

I think 加算 is often translated as "premium" in the context of health insurance. Based on the treatment and condition, each criterion has predetermined points, which will be converted to medical fees (1 point = 10 yen).

disagree  4carnations: No, it's not "just" a premium. The premium itself refers to the rate you pay on a regular basis. https://www.investopedia.com/health-insurance-premium-477314... This is a special additional cost.
