è tratto da un articolo medico "....quando un paziente ha già dei figli, spesso si manifesta un senso di "sospensione" per la possibilità di aver trasmesso loro una " falla genetica". Nel caso, invece, il compito procreativo non fosse stato ancora espletato, al senso di colpa si unisce quello di perdita....."

praticamente, il significato è "in caso il paziente non avesse messo su famiglia", "non avesse ancora procreato": il problema è di come renderlo in modo formale!
non posso, visto il tono del contesto, scrivere "....hasn't had any kids yet".

Explanation:
This "psychology" is highly specific to a particular philosophy in Italy (compito in the sense of a task, a duty, an obligation). A simpler way of putting it might be to say "if they don't already have any children", but this would remove the ideological overtones which may not be consciously present in the mind of the author (merely using the "normal" phrase with no attention to the underlying sense). A thorny one!
