Hi, I am not sure that it is a payment form. It seems to me that it means: Cortese Attenzione bancaria (della banca). If that's the case the translation is : Attention (attn): Bank (To The Attention of the Bank). I did not translate "Cortese" because usually you find only Attention, but if you want you can write "Kind".

As I write I am thinking it might also mean CONTO ATTESTAZIONE BANCARIA (BANK ACCOUNT STATEMENT) but I have never heard of it as payment form.
