domestic partnership / cohabitation relationship
Explanation:

live in a shared household

Explanation:
I hereby verify with my signature that X and Y constitute a couple and that they have been living together in a shared household at least for three years.

In case it's a legal document being signed, "certify" instead of "verify" is better.

neutral  juvera: Shared household could mean mother, daughter, granddaughter living together, or anybody else you care to mention. Besides: is there a certificate for cohabitation? That's precisely what is missing from the relationship mentioned!//Not certify, CONFIRM!
19 hrs
-> There must be a certificate if someone signs to certify.
