Explanation:
this sentence makes no sense.
epithelial cells, of any kind, are not seen in a peripheral blood smear.

is the sentence describing a path report on a urine sample?
maybe what is meant is: few RBCs were seen, as well as transitional epithelial cells......

I would have to say that the "not rich" is shorthand for:
few to small number of cells.
thus:
cytology examination of the urine / urine cytology revealed a large number of red cells and / but only a small number of transitional epithelial cells and ......
