I have no idea what any of it means. The Yad Eliezer example cited above has 9 digits and begins with 580. Israeli ID numbers have 9 digitis. You could try googling מלכ"ר מ.ה followed by the number in your document and see what you come up with. If you don't find anything, you could just write "Non-profit number followed by the five digits. I don't think it makes sense to guess beyond that if you really don't know what to call that number (ID, Tax ID, Amuta, Registration Number, Association Number, etc. Good luck!

I am guessing that this is the registration no. in מרשם העמותות.
The abbreviation "מ"ה" not infrequently stands for "mispar ha... whatever", according to the logic of the secretary who typed the form.

The reg. number is reqd on all forms (unlike the tax ID which, incidentally, may well be identical).
