"Wie zufrieden sind sie mit der Störungsannahme durch... ...die Service-Leitstelle von ABC bezüglich:

- Erreichbarkeit der Leitstelle
- Freundlichkeit der mit Arbeiter
- Aufnahme des Störungsverlaufs?"

From the "Service" section of a customer satisfaction survey. (Earlier theories were that "Service" might be "after-sales service", but it seems to be more along the lines of troubleshooting and maintenance, so maybe "servicing".) Those completing the survey have to check on a scale of 1 to 5 how true certain statements are for them. I don't understand what is meant by "Aufnahme des Störungsverlaufs", and can't find a translation for "Störungsverlauf" anywhere, or even one single reference to the word, which is obviously made up. TIA for any light you can shed on it.
