I understand the concept - just can't put my finger on the English word for it. It's in the context of psychological rating scales for things like depression or hyperactivity - a Selbstbeurteilungsfragebogen is where you rate yourself according to the questions asked, a Fremdbeurteilungsfragebogen is where someone else rates you.

Thanks to everyone. I initially liked Kirsty's "third-party evaluation", but on reflection that won't do, because there aren't three parties involved, only the subject and the tester! So external assessment questionnaire it shall be.

agree  Kirsty Mason: Personally prefer third-party evaluation, suggesting it is undertaken by a neutral/impartial 3rd party rather than external (internal/external could be confused - evaluation of external symptoms, or completed outside the clinic)
