attachment figure

Explanation:

agree  Rowan Morrell: This one is suggested by Leo, along with "psychological parent". But in this context "attachment figure" is probably the better option.

soul mate

Explanation:
Someone with whom you can easily relate. It is neither a role model nor a surrogate parent, and may or may not be spiritually related. It is someone whose hand you can hold with confidence when things become difficult. Not a tag along.

counselor

Explanation:
We don't even know what the therapy is, so I'm reluctant to get too clinical/psychological about it. I'm not convinced "attachment" in a deep psychic sense is even desirable.

agree  Edhild: this should sound professional - important seems to be the statement, that the same counselor will be in charge for the duration of the treatment

Probably somewhere between a "patient companion" and a "contact person"

Explanation:
I think Johanna's generally right but not in this context.

IMO an "attachment figure" is a mother, father etc. - someone with whom one builds up a psychological attachment over a period of time.

The "Bezugsperson" in the question appears to an individual who is assigned to the patient by the hospital (to liaise with the patient and so on).
