I believe معالج here is just a redundant adjective for پزشک. If you write "Physician:" on a patient form and then put the name of the doctor responsible for that patient, then everyone will know that this is the doctor actually responsible.

The doctor's job is to treat patients, I think the one in charge is what you need here, because it is redundant to say the treating doctor.
