Crossover means that due to mistakes (wrong tank container, wrong connection at the plant, etc) the wrong product is loaded or unloaded in the customer's tanks. The result is that two products are mixed up.

For what it's worth, we generally call that contamination. Common causes are: plant staff connecting the wrong hose (or instructing the driver to connect it, which is against plant procedure - pointing a finger at one of five hoses is hardly SOP), improperly cleaned tank containers or hoses.
