The first Form (in "Choice of Schools Form") refers to "formulir"/"borang" (if you prefer not to use kata serapan). The second Form (in "Form One") actually means "Kelas" in British education system hierarchy, which I believe may still be in use in some schools in Hong Kong. i.e. Form Five actually means Kelas Lima (SD).
