A patient who is admitted to hospital for observation for a possible stroke has a list of tests, etc. ordered, e.g.
ECG, chest X-ray, an ultrasound of the carotids, telemetry, a physio and occupational therapy assessment, an SIP score and a "DAB-skema".

Can anyone tell me what type of form this is please?

Explanation:
Is this a misprint or mis-hearing of a DAP-skema? If the medical notes have been registered from speech recognition, the difference between DAB and DAP is not easy to catch.
