Asimismo, todas las palabras del segmento propuesto están en mayúsculas por lo que parece evidente que se trata del nombre completo del formulario. Yo diría que se trata de un formulario que debe cumplimentarse en el caso de viajar al extranjero, por ejemplo, al objeto de poder recibir atención médica en el país de destino.
