Certifico que el (fecha), el Rector de la Universidad XXX otorgó a doña María Pérez el grado de LICENCIADO EN MEDICINA VETERINARIA y fue aprobado con 4,9.

The wierdest wording ever, isn't it?

My try:
...BACHELOR'S DEGREE IN VETERINARY MEDICINE, which Ms. Fulana de Tal obtained with 4.9 on the 1-7 grade scale.

Obviously, this could be translated several ways (and each one might better/more cohesive than the original).

Maria Eugenia, your take on it would certainly work regarding "that was obtained;" another way would be "and it was obtained ..." But in the case of using the conjunciton "and" it would be necessary to use "it." (pun intended)

Ahora bien, no está nada de más senalar que la escala de calificaciones en Chile es de 7 puntos, pero es materia de una NOTA de la T., no se debe incluir en el texto porque no aparece. Eso, siempre que la escala de calificaciones no aparezca en otra parte del documento.
