I'd appreciate someone confirming that I've understood this correctly, because it's important. My understanding is that the father's name was introduced on the birth certificate and then WITHDRAWN(??). Has it been the case in Brazil in the past that the birth certificate did NOT include a field for the father's name? If it was briefly introduced and then withdrawn, how can they know that neonatal deaths are higher when the father is not declared?

A mother gets the birth declaration in the hospital, takes it to a notary and obtains the birth certificate there. The last line of the birth declaration form states, in big block letters, that it is not a birth certificate. See http://portal.saude.gov.br/portal/arquivos/pdf/declaracao_na...

The procedure is similar at the other end of life: you get the "atestado de óbito" in the hospital and then the "certidão de óbito" at the notary. You need the latter for legal purposes; e.g. claim an inheritance, life insurance, etc.

Explanation:
I confirm that you have understood the text correctly. It does in fact state that the filed for the fathers's name was included in the "DN" and the later removed or withdrawn from it.
There's no reference to whether birth certificates have or have not been included the father's name field in the past nor is there any injdication tohow they know that neonatal deaths are higher when the father is not declared.
