1. 1. Country Profile
The Federal Republic of Nigeria is found in the West of the African continent. It is boarded to the North by Niger, to the West by the Republic of Benin, to the East by the Republic of Chad and Cameroon, and finally the Atlantic Ocean and the Gulf of Guinea provide sea access in the South. Nigeria, like the vast majority of African states, is a colonial creation, in the sense that the British have forced the union of three regions which they ruled at different times. In 1906 the colony of Lagos and the two protectorates of Western and Eastern, were united in the Colony and Protectorate of Southern Nigeria.
On the date of October first 1960, Nigeria gained independence as a constitutional monarchy. The queen of England remained the supreme head of the state, but the Governor-General, his representative in Nigeria, was no longer a senior British official, but a Nigerian. The first federal election went to the Nigerian National Alliance 198 of the 253 seats in the Federal Parliament: basically the conservative block had won the majority of two-thirds in the House.
In the night between 14 and 15 January under the guidance of K. Nzeogwu came the alleged “coup of the biggest five”, but the military coup failed to take control of the whole country. In the North and the South-West they were successful in decapitating the regional leadership; in South-East and Mid-West the prime ministers were arrested, but released shortly afterwards.
The innovative policy of Ironsi consisted of the abolition of the Federation. Nigeria would become a unitary state. Yakubu Gowon knew that the counterfoil of all Nigerian turbulences was his federal asset. On May 27, 1967 the assembly of the Eastern Region proclaimed independence, for the secession of the new state called the Republic of Biafra. That same day Gowon announced the state of emergency throughout the country, and the abolition of the regions and the creation of twelve states. On May 30 Ojokwu officially announced the birth of Biafra, seen from the federal summit as an act of open rebellion. On July 6, 1967 began armed conflicts that lasted until January 12, 1970, when Biafra surrendered.
The main result of the Biafra war and the new federal structure was the beginning of a political centralization. Nigeria had turned into a centralized federation. Created without any consultation from the center and not having the size and resources of older regions, the new states were expected to become increasingly dependent on the center for their financial survival and the determination of their political leadership. On July 29, 1975 the regime of Yakubu Gowon was pushed aside by a military coup, this time with the approval of the population. The coup was the work of a group of soldiers who channeled the will of the population weary of the Gowon regime and the incomplete transition to democracy. General Murtala Mohammed was made the head of state, accompanied by his right arm, General Olusegun Obansajo. Mohammed Obansajo led reforms to restructure the Federal Military Government; purge the bureaucracy, the judiciary, the parastatal equipment and the universities to further centralization of federal power and the extension of state control over society and economy.
Between July and August of 1979 the elections for the Senate, the Federal House of Representatives, the parliaments of the States, the governors of the same, and finally for the presidency took place. The results meant the victory of the National Party of Nigeria, which emerged as a majority party at the federal level. At the presidential level, the candidate of NPN, Shehu Shagari, was elected. Even the elections of 1983 are again a story of serious violence, irregularities and fraud on all levels. Shehu Shagary was re-elected for president by getting the vast majority of votes. The country, after the elections, was found next to anarchy. On December 31, 1983, the army was called openly for five months by different sectors of society, intervened to put an end to the Second Republic. On August 27, 1985, after yet another coup, Ibrahim Babangida came to power and, far from wanting to continue the transition to democracy, consolidated his military dictatorship and assumed to himself the highest powers.
The return to democracy in 1999 under President Obansajo came after years of transition that took place after the cancellation of 1993 presidential elections the establishment of a military government led by General Abacha, marked by years of brutal dictatorship, civil and political violence.
Today, Nigeria is Africa's most populous state, with 152,217,341 inhabitants, a density of 96.3 persons per km2, and a growth rate of 1.96% in 2010. 41% of the population is under 15 years, while only 3.1% of the population is over 65. The Nigerian population in 2010 is predominantly rural, while 48% live in urban areas.
Nigeria is a Federal Republic with a presidential-style system of government. It has a tripartite structure of government, formed by the Federal Government which consists of 36 states, to which the Federal Capital is added (Federal Capital Territory) that forms a state itself.
The 36 states and the FCT are divided administratively into 774 local government areas. States are grouped into six geo-political areas: Northeast, Northwest, North Central, Southwest, Southeast and South-South then. Nigeria is a country ethnically and religiously complex, which encloses more than 400 ethnic and linguistic groups, and whose citizens practice different religions and come from different cultural backgrounds. Muslims represent 50% of the population, while Christians make up 40%, the remaining 10% consists of religions, or beliefs of native background.
Nigeria is the fifth country in the world in oil production, which dominates the local economy, accounting for 95% of exports. In 2009, Nigeria has recorded a Gross Domestic Product amounted to 339 billion USD, an increase of 6% over the previous year, and that a per capita income is around 2.300 USD. Nigeria is the 33rd richest country in the world, but in the ranking of per capita income it is located at 184th place instead. 74% of the population lives below the poverty line.
The development in Nigeria is very complicated, and is the result of nefarious policies, based on oil that have been developed over the years and that have caused so much success because of the failures of the numerous military regimes that succeeded one another over time. Nigeria occupies the 142nd position in the Human Development Index, ranking among the 30 poorest countries in the world .
1. 2. Nigeria and HIV/AIDS
Unlike the region of Eastern and Central Africa, where first cases of HIV/AIDS were identified in the early eighties, the first case ever reported in Nigeria was in 1986. This case was diagnosed in a Ghanaian prostitute who had practiced his activities in Côte d'Ivoire .
At the beginning of HIV/AIDS epidemic, the world's attention was directed especially to sub-Saharan African countries such as Tanzania, Zaire, Uganda, Kenya, Malawi and other countries of Central and Eastern Europe. In southern Africa the attention was directed to countries such as Namibia, Zimbabwe, Botswana and South Africa later. These areas were seen by most international public opinion as the outbreak of the epidemic, and they recorded the most cases of infection, while the West African region was generally considered free and not yet "infected" by the virus.
The general impression at the beginning of the epidemic saw Nigeria as one of the last countries to be affected and infected by HIV/AIDS.
The low level of literacy, a strongly backlogged health care system, the difficulty of access to care by the poor population in Nigeria, along with traditional medical and hygienic practices, made the problem of HIV infection pass underhandedly by the people and the government. Thanks to the military regimes that followed one another during the period when the epidemic was growing in Nigeria, the government's initial attitude towards the population during the epidemic was to deny its existence .
Being the most populated country in Africa, Nigeria accounts for 20% of people living with HIV/AIDS throughout Africa and 10% worldwide. Within two decades, HIV/AIDS in Nigeria has become a "generalized epidemic" and current data suggests that this situation is still at an emerging stage, and is far from being resolved or at least contained. .
Statistics, dating back to 1991, show a constant increase in the percentage of Nigerians infected with HIV. If in 1992, the percentage of infected people in the Nigerian region was 1,8%, in 1994 the percentage was doubled, to reach a peak of 5.8% in 2001, which was followed by a slight fall. The latest figures available show that, in Nigeria, people living with HIV/AIDS are 2.98 million, with a percentage of 4.6%. In 2009, deaths due to AIDS were 192,000. One of the most important economic and social impacts caused by the impact of HIV/AIDS is the constant increase of the orphans of parents with AIDS, estimated in 2009 around 2.175 million. People living with the virus for over 15 years are more than 2.4 million (3.6%) while those for less than 15 years averaged about 220,000 cases.
With an average of 4.6% in 2008, it is estimated that people living with HIV in Nigeria are 2.98 million. It is, in numbers, the second largest epidemic in the world after that in South Africa, and the largest in the West African region.
At the regional level there are lower rates of infection in the South-West (2.0%) while the rate is highest in South-South (7.0%). At the state level, the percentage varies from 1% (as for example in Jigawa State or Eka) to 10.6% (recorded in Benue State), with a higher prevalence among women and girls.
2. 3. AIDS related to regional and age-group factors
Looking at the map of Federal Nigeria, the first thing that can be noticed is how all 36 states and, with the addition of the FCT, have recorded cases of HIV/AIDS. This obviously means that there are no zones or areas where the epidemic has not spread. According to the latest reports available, dated 2007, the National HIV/AIDS and Reproductive Health Survey (NARHS) in Nigeria, the disease is higher among women (4.0%) than men (3,2%), as it is higher in urban areas (3.8%) than in rural areas (3.5%). Whether in rural areas than in urban areas, it is always women who experience the greatest number of infections. These are the highest among the age group of 30 to 39 years (5.6%) and lowest in the age group between 15 and 19 years (1.7%). Every day approximately 6000 young people between 15 and 25 become infected with HIV .
In developing countries it is estimated that about 1/3 of sexually transmitted diseases is found in people between the ages of 13 and 20 years. Research shows in Nigeria that the first sexual intercourse takes place between 10 and 19 years, and an average of 17 years.
According to the report by NARHS, there are more cases of HIV (4.6%) among those who have been educated to the third degree of study (high-school) rather than those who have not received any formal education (2.7%). Looking at the map, we can deduce that some areas may be considered more risky than others. The reason is still unclear, perhaps nonexistent, also because we must consider the significant migration between the different states in Nigeria, where so it happens that a person may have become infected outside of their home state but later had to re-enter as a result of being HIV positive.
The State of Ekiti in Nigeria's South-West shows the lowest level of people living with HIV/AIDS (1%), while the State of Benue in central area of the North records the highest rate (10, 6%). We may add that the local area of Bwari, located in the FCT, is the site that shows the greatest percentage reaching 22%.
The report shows that women who have received an education of first, second or third level, show rates of HIV/AIDS respectively equal to 5.1%, 5.8% and 4%. Single women living with HIV are 5.9% versus 4.7% that of married women. Yet again, 5.4% of people living with HIV live in the North Central zones, and they differ between urban areas where the rate reaches 6.2%, and the rural area where it reaches 4%. The area of the North West recorded a rate of 2.4%, 2% in rural and urban areas by 2.6%. The area of the South East recorded a rate of 3.7%: 3.4% in rural areas and 5.4% in urban areas. The South West stands at 2%, and it can be seen that in rural areas the rate of HIV prevalence is 1.7%, versus 2% in urban areas. Finally, in the South, the rate of HIV prevalence is 7%, of which 4% is in rural areas and 7.1% in urban areas .
The latest figures available, dating back to 2008, show that Nigeria’s age group of 25-29 years recorded the highest rates of infection, while the percentages are reduced considerably in the age group of 40-44 years, which is the age group with the lowest number of people infected.

HIV/AIDS Integrated Biological and Behavioral Surveillance Survey (IBBSS) was conducted in 2007 among the population whose behavior or work exposed them to a greater risk of contracting sexually transmitted diseases. These groups include prostitutes (FSW, female sex workers), men who have sex with other men (MSM, men who have sex with men), intravenous drug users (IDUs, injecting drug users), employees in the transport field (TW, transport workers) and those in the armed forces and police. Despite the prevention and control programs specifically aimed at these subgroups, and although the number of infections in Nigeria is decreasing, the epidemic among these subgroups is increasing.
It is seen that the greatest number of infections among prostitutes is found in brothels (37.4%), followed by street prostitutes (30.2%), MSMs (13.5%), IDUs (5.6%), the TWs (3.7%), police (3.5%) and the armed forces (3.1%) .
Prostitution is illegal in Nigeria, but more than a million girls work in brothels or on the streets. Since this activity is illegal, the sex trade is carried out mainly in the suburbs of the city. For many prostitutes, this is a temporary work just to raise enough money and be able to become involved in other jobs or activities later on. Prostitutes appear to be the subgroup most affected by HIV/AIDS. In some states, you can see that the rates of infection within this category are increasing, such as in Kano, up to 49.1%. Alcohol abuse is often associated. The state of Lagos records the lowest percentage, 23.5% among brothel prostitutes, and 12.9% among street prostitutes; an important point when one considers the fact that in this State the highest percentage of condom use is registered (98.2% among brothel prostitutes, and 98.6% among street prostitutes). The research also shows how, among the prostitutes the use of condoms with their partners is lower (38.1% among brothel prostitutes, and 46.1% among street prostitutes), and yet surprisingly reaches high levels with random partners (83.3% and 84.8%).This means that the risk of infection is higher for sex workers’ partners rather than their customers, making it a potential bridge for the widespread of the epidemic among a population that is not enclosed in any groups, that is the general population .
Among the group of men who have sex with other men, the percentage of infected stands at 13.5%. This group consists largely of children under the age of 25, which represent 75% of the total . The MSMs show a high level of awareness about HIV prevention, yet the consistent use of condoms is less than the percentages recorded in the group formed by the prostitutes. It is a group that still has only a few prevention programs addressed towards it. The non-constant use of condoms can be of course a risk factor for the spread of this disease among the general population, especially when the members of this group have unprotected sex with their female partners.
The data of HIV / STI Integrated Biological and Behavioral Surveillance Survey (IBBSS) of 2007 show that those who use intravenously-injected drugs represent the third most affected group by HIV/AIDS. Also this subgroup, such as MSMs, is composed mainly of youngsters, of which 60% are sexually active. Within this group, only 40% use sterilized material during injection of the drugs .
Among the groups at risk, the categories that appear to be least affected by the epidemic are the workers of the transport sector, the armed forces and the police. As the table shows, the so-called "risk groups" instead represent only 31% of new infections. This data demonstrates that they are not actually at risk, but perhaps exposed to risky behavior.
2. 5. Factors for the spread of HIV/AIDS in Nigeria
2.5.1 Social Factors
Although HIV/AIDS knows no social status, ethnicity, race and religion, nevertheless it can be said that some people are more vulnerable and prone to infection than others.
Nigeria has long been considered a country where the HIV/AIDS was negligible, which according to other regions and other African states, considers long-itself as a country free from this type of virus and disease. Despite this fact, many experts warned the country about the danger of this complacency. Rather than preparing the field and informing people about the dangers of this epidemic, there was a total denial of the problem by the government and the intellectual class, and therefore by the general population.
It is known that despite the complaints from doctors upon the appearance of first diagnosed cases of showing symptoms similar to those found in AIDS patients, the Nigerian Federal Health Authorities continue to vigorously deny its existence.
The reasons for this behavior remain unknown. There was a general lack of acceptance of how the epidemic was real and how it could easily be contracted by anyone just through sexual intercourse. This led to repeated denial of a passive stance of the majority of the population who thought it was a disease in the West. The World Health Organisation (WHO) in 1958 defined "health" as a "state of complete physical, mental and social welfare and not merely the absence of a disease ”. From this definition, we can understand how health in the human being goes beyond the mere biological function of the body. In this context, health is considered more than a means to an end and can be defined as a resource for everyday life that allows people to lead a productive life as individuals, on a social and economic level. This definition recognizes a link between the health of the body and health of the person, and not only in the context of their own individuality, but in relation to the whole community. To be healthy, a person must interact with its environment and maintain themselves within that environment.
From this definition of health, it is clear that the social unrest, economic and political conditions of Nigerian society are related to the disease, and particularly with the rapid spread of this.
Since the early eighties the new millennium, Nigerian hospitals had been reduced to mere consulting clinics. The health services and basic preventive measures, such as a basic health education, provision of clean water, health care for pregnant mothers and unborn children, medical supplies, adequate care to common diseases, proper nutrition, and so on, lacking in most hospitals, and to this day, despite slight improvements, remain a serious problem, especially for the lower classes. .
An inadequate health care system be seen in the blood transfusion domain. It is a known fact that the transfusion of infected blood has contributed greatly to the spread of HIV /AIDS. In Nigeria, most of the hospitals do not have the equipment for blood testing, partly because of high costs of a process such as that of the transfusion. It is therefore not surprising that infected blood can be transfused to innocent people and is especially unexpected. .
Connected with the problem of an inadequate health system, there is the question of the economy. Several scientists have historically confirmed the importance of health regarding the economy, and in particular the connection between poverty and health, so the effect of the economic regression in developing countries affects not only the economic sector but all sectors. For a poor economy it will be more difficult to achieve effective health policies, and in turn more expensive, as well as research funding.
It is no coincidence that between 1992 and 1998, a period in which Nigeria experienced a dramatic increase of HIV/AIDS infections, the general situation of the country was in a state of chaos, which fueled the spread of the infection itself. In situations in which political freedom is suppressed, as during the military regimes of Shehu Shagari and Sani Abacha, is not difficult to understand how the right of citizens to access health care was actually a nonexistent issue for the political class.
In the health sector, a major problem but often not considered is the difficulty of finding medical personnel who can work in a few and sometimes crumbling hospitals. One factor that makes it difficult to find such staff is the emigration of doctors and nurses. This is due to the fact that the majority of doctors and nurses come from urban areas and thus are unlikely to work in rural areas.
Rural areas are characterized by the absence of sewerage, severely limited access to electricity, primitive social services, poor quality of school facilities and roads, a fragile health system and inadequate access to clean water. Under these conditions, the number of medical staff and hospital should at least be doubled to ensure that the population has access to basic health care .
Finally, there is also the problem of the economic capacity of the population to face the costs of health services. If on one hand, the constant low quality and inadequacy of public health services has made it almost a forced choice to contact one’s private health sector, unfortunately on the other hand, one must also consider that over 74% of the population lives below the poverty line or less than 30 USD per month, making it impossible to access health services . Even where there is access to public health services, many people are not able to handle these minimal costs. In Nigeria, to date, tens of thousands of people continue to die from diseases that are easily treatable.
It is clear that the Nigerian health system is as fragile as inadequate. No country can maintain a stable economic growth in the absence of an adequate health system. The problem of health care is an enigma in Nigeria and its citizens are the first to suffer the consequences. Solving the problem requires an aggressive approach by the Federal Government, State and local governments, who must work in a coordinated manner. The problem of health care is a national emergency and as such must be considered by the Government in all its ramifications. With the devastation caused by HIV/AIDS and other preventable diseases, Nigeria must get to work and carry out public health programs to prevent future disasters .
Stigmatization is a social phenomenon that attributes to a negative connotation to a member or a community group, in a downgrading way . This phenomenon has certainly contributed to the spread of the epidemic in Nigeria as in other parts of the world. The stigma is based on prejudice, which means that anyone carrying the virus is in many respects doomed. When a person is identified as HIV positive, they are placed automatically by the community under interrogation regarding their sexual behavior, which thus tends to condemn. Unlike diseases such as diabetes, cancer, or heart disease, AIDS carries with it the stigma of the disease. This is explained by the fact that in most cases the mentioned diseases are inherited and not acquired by the patient through direct actions such as AIDS.
The danger of stigma is twofold in that it leads a large number of people who are probably at risk of infection, not to want to take the test for fear of positive result, then suffering social, economic, and familial consequences, for being the bearer of this disease. In this way, creating a vicious cycle as people potentially affected by the virus could put others at risk of infection through harmful behavior towards themselves and towards others, the most common of which is to have unprotected sex.
The stigma is closely intertwined with the cultural constructions. Often, some diseases have been criticized as a result of witchcraft. AIDS is not an exception in this case.
Urbanization and migration of the population were factors that contributed to the spread of HIV / AIDS in Nigeria. Migrants were, and are, above all men, sometimes married, that leave their wives in rural areas and seek work in urban areas. The result was that these men often engaged relations with other women in the places where they worked, while others sought entertainment through the exploitation of the prostitution market. Once back home, in rural areas of membership, it was easy to spread the virus among their wives. Although, as we have noted, the relationship between infections in rural and urban areas is fairly balanced, it is still strongly established among the population the idea that the greatest number of infections is located in urban areas.
We should add that not all people who migrate to urban areas work have luck finding one. This is especially true for women, who are thus "forced" into prostitution as the only means of livelihood. A job that is certainly the most lucrative in the cities and urban areas with high population density, but at the same time facilitates the rapid spread of the disease.
2.5.2. Economic Factors
As you might guess, the economy plays a central role in the spread of HIV / AIDS, as throughout the whole world, as much as in developing countries. The disease appears to be concentrated among the population that is already marginalized both socially and economically.
The Nigerian economy has experienced a steady deterioration in the last three decades, where it recorded slight increases or decreases in its gross domestic product, and an enormous debt that cripples any chance of growth.
The most striking effect economic difficulties of the country, and the mismanagement of these is that despite economic growth in 2009 stood at 6.1%, a GDP of 173.3 billion USD, which makes Nigeria the 33rd richest country in the world, the population living below the poverty line on less than 1.30 USD per day, is 74%.
Widespread poverty is not just about disadvantaged and "rural classes", but has also struck what once was considered the middle class. The Nigerian society is divided into extremely poor and extremely rich classes, with virtually no middle class. There is a huge disparity between the two classes. Because of this disparity, we can note a strong inequality of resources. The vast majority of the population lacks access to the most basic means of livelihood, such as good nutrition, access to drinking water, housing, education and health services. Poverty is so strong that many of these people cannot even afford to pay for doctor visits. Therefore, do not go even to hospitals. Many families are thus compelled to send their children, especially younger girls, to work on the street, in bars and hotels; it seems to bring more, if not only available resources needed to support the family.
This situation was created in the period of the dictatorship of General Sani Abacha. It was at this juncture of history that the Nigerian economy fell apart, and the country lost its international image. It was around this time that Nigeria experienced a ferocious increase in HIV infections. It is therefore no coincidence that HIV / AIDS and poverty are closely related .
The cost of treatment for infected people, has certainly contributed to the spread of the disease. In the past, the lack of drugs or their exorbitant cost, however, has been a nightmare for people living with HIV/AIDS, despite efforts by the government to make treatment more accessible.
Some data dating back to 2003 may help clarify the idea of the impossibility of access to treatment by the majority of the population. Antiretroviral therapy, at the time, had a cost of approximately 10,000 USD per patient per year. Considering that the average salary of a Nigerian is around 660 USD a year, we understand the inability of access to care. It is clear how a patient with HIV/AIDS cannot face the expense of buying the drugs, because they will still have to think about basic needs such as food and other basic necessities. This means that until a few years ago there was no hope for people living with HIV in this country. Today, Nigeria has made great steps in accessing medical care for patients with HIV/AIDS, starting to produce medications in the same pharmaceutical companies in Nigeria, leading to a cost reduction from 240,000 naira to 60,000 naira per year (about 400 USD). .
One of the consequences of economic hardship in Nigeria is the loss of jobs, especially in the field of unskilled labor, which has led many businesses and companies to cut down the number of workers. The consequence has been an exponential growth in the number of unemployed people in the country (the unemployment rate is around 28%). Linked to the unemployment problem is the growing phenomenon of street selling, which has become one of the major features in most Nigerian cities. This practice is common in bus stops, parking lots, rest areas on major roads, traffic lights and traffic. These vendors are especially young girls, many of which have, however, if not an average-to-high education- , then at least elementary. Many of them had to leave school because of poverty or other reasons. These vendors contribute to the spread of HIV/AIDS because often these girls do not just sell their goods, but also their bodies, to increase sales of low income.
Prostitution plays a key role in disseminating HIV/AIDS in Nigeria as well as generally throughout Africa. Prostitution is naturally linked to poverty.
The prostitution market is strong especially in towns and larger cities of Nigeria. This is due to the fact that there are in the cities, as well as a greater number of people and therefore potential customers, such facilities like both large and small hotels, bars, nightclubs, where the young Nigerian can offer sex for money.
2.5.3. Political Factors
As well as social and economic factors, political factors have and continue to play a significant role in the spread of HIV/AIDS in Nigeria.
As mentioned earlier, there was reluctance in the eighties by the government to admit the presence of HIV/AIDS in the country. Consequently, the population was totally unprepared and uninformed about the damage of what had long been an epidemic in the country and was spreading rapidly among unwary population. It was not until August 23, 1991 that the first official campaign against HIV / AIDS was launched by the Federal Government.
HIV/AIDS was viewed solely as a public health problem, and never through a multisectoral approach. One such approach would mean that different ministries, starting from the Ministry of Health, would be triggered in the campaign against HIV/AIDS; would also have meant a coordination between different branches of government to each petition, including non-governmental organizations as well as the academic community. Since the independence from Britain, declared on October 1, 1960, the military has dominated the political life of the nation. The military has ruled from 1966 to 1979 and then 1984 to 1999. Authoritarianism followed the exploitation by the rich class and had marginalized. Moreover, military regimes create instability and underdevelopment.
Historically, military regimes and rules imposed by them have caused havoc for the entire country. These ravages, which have implicated the social and economic fields, have contributed to the spread of HIV/AIDS. Examples of the negative effects of military dictatorship are easy to find. It is no coincidence that the public health began to deteriorate during the dictatorship of General Ibrahim Babangida, who was head of the military regime from August 1985 to August 1993. It was during these years, for example, that there was substantial emigration of doctors and nurses from Nigeria to Saudi Arabia and other parts of the world searching for job opportunities. This mass exodus represented the lack of job satisfaction caused by measures and policies taken by Babangida. Babangida's successor, General Sani Abacha, who was head of the military regime from November 1993 to June 1998, only served to worsen an already disastrous situation. His government was marked by a lack of political stability, from tyranny and oppression in every corner of the country.
The mass media, which could play an important role in information for the battle against the threat of HIV/AIDS, was constantly harassed, under surveillance and under attack. Many journalists disappeared mysteriously while large numbers were imprisoned or sentenced to hard labor. As a result of oppression and authoritarianism, in sexual pleasure, many people sought an escape from the reality of the country. The result was the alarming number of people who were infected by the virus . The raid of the military in political life created instability that was reflected in the significant increase of HIV/AIDS infections .
Corruption has always been one of the factors that characterize all the administrations that have occurred in Nigeria. To date, according to the classification of the organization "Transparency International", Nigeria is at 134th place on the index of perceived corruption . In other words, Nigeria is a country where corruption is widespread and has become an embarrassing phenomenon that is consistently associated with the country. The corruption is visible everywhere, between the country's leaders as well as between the school desks. The effects are traceable in a huge national debt, crumbling infrastructure in every sector and especially in the health and the fact that the majority of the population remains stuck in extreme poverty.
Corruption at all levels contributes to the spread of HIV/AIDS to the extent that those resources could be used to relieve the suffering of the people. Corruption in Nigeria has reached such levels that even the resources allocated for the fight of HIV/AIDS through the Petroleum Trust Fund (PTF) was most likely badly managed . The acquisition of blood test kits purchased through the PTF in January 1999 was immediately criticized. The PTF announced the first national conference on AIDS held in Abuja in 1999 that kits for blood analysis should be distributed in order to diagnose HIV in every hospital and other secondary health facilities. But the doctors and staff who work daily in the field argued that the enormous expenses faced to provide health facilities of these machineries were foolish. Provisions for prevention campaigns at the most vulnerable segments of society and medicine were at that moment in history the most important if not fundamental. The criticism was even stronger as the time the PTF was the only agency that provided funding for indigenous fight HIV/AIDS, which were regularly used in a very timely, if not even wasted, way. .

Only the admission of some soldiers suffering from HIV/AIDS has raised the level of awareness of the impact from peacekeeping operations in the spread of the epidemic .
2.5 4. Cultural Factors
There is no doubt that some cultural practices are harmful and potentially complicit in the spread of HIV/AIDS in the country. It is no coincidence that today there are more women living with HIV/AIDS than men. Of the 22.4 million people in 2008 who lived in sub-Saharan Africa with HIV/AIDS, 57% were female. To date, in Nigeria, of the 3 million deaths due to AIDS, 1.61 million were women. In 2009, the number of women who died of AIDS was 106,000, against 88,000 men .
The explanation is that today, women continue to be viewed as inferior beings than men. In most cases, in the African cultural tradition, women are always to be the victims. It is no coincidence that if husband and wife can keep a secret that one or both are affected by HIV/AIDS, marriage continues to last, but if the news leaks out that the woman is affected then she will be the only one being marginalized if not expelled. This does not happen when the news concerns her husband. Nigerian culture always considers women responsible for the spread of the virus within the family.
The alleged inferiority of women is expressed in cultural practices such as female genital mutilation, which puts at risk of exposure to the virus young African women, as well as being an inhuman practice. For example, it was reported that the use of same blades and knives for cutting genital areas of young Nigerians in the traditional therapy of female circumcision is a possible route of infection. Through this practice and contact with blood, it's easy to become infected and infect others. 1. 1. Profilo del Paese

La Nigeria, come la stragrande maggioranza degli Stati africani, è una creazione coloniale, nel senso che furono gli inglesi a forzare l’unione dei tre territori di cui avevano acquisito il controllo in epoche differenti. Nel 1906 la colonia di Lagos ed i due protettorati del Western ed Eastern, vennero uniti nel Colony and Protectorate of Southern Nigeria.
Alla data prevista, il 1 Ottobre 1960, la Nigeria acquisì l’indipendenza come monarchia costituzionale. Il capo supremo dello Stato continuava ad essere la regina d’Inghilterra, ma il governatore generale, suo rappresentate in Nigeria, non era più un alto funzionario inglese, bensì un nigeriano. Alle prime elezioni federali alla Nigerian National Alliance andarono 198 dei 253 seggi del Parlamento federale: in pratica il blocco conservatore aveva conquistato la maggioranza dei due terzi alla Camera.
Nella notte tra il 14 ed il 15 Gennaio sotto la guida di K. Nzeogwu avvenne il cosiddetto “golpe dei cinque maggiori”, ma i militari golpisti non riuscirono a prendere il controllo di tutto il paese. Nel Nord e nel Sud-Ovest riuscirono effettivamente a decapitare la leadership regionale; nel Sud-Est e nel Mid-West i primi ministri vennero arrestati, ma subito rilasciati.
La politica innovativa di Ironsi consisteva semplicemente nell’abolizione della Federazione. La Nigeria sarebbe diventata una Stato unitario. Yakubu Gowon sapeva che la matrice di tutte le turbolenze nigeriane era il suo assetto federale. Il 27 Maggio 1967 l’assemblea della Eastern Region proclamò l’indipendenza, per secessione del nuovo stato chiamato Repubblica del Biafra. Lo stesso giorno Gowon annunciò lo stato d’emergenza in tutto il paese, nonché l’abolizione delle regioni e la creazione di dodici Stati. Il 30 maggio Ojokwu annunciò ufficialmente la nascita del Biafra, vista dal vertice federale come atto di ribellione aperta. Il 6 Luglio 1967 iniziarono gli scontri armati, che si protrassero fino al 12 Gennaio 1970, quando il Biafra si arrese.
Il risultato principale della guerra del Biafra e del nuovo assetto federale fu l’inizio di un accentramento politico. La Nigeria si trasformò in una Federazione centralizzata. Creati dal centro senza alcuna consultazione e non avendo le dimensioni e le risorse delle vecchie regioni, i nuovi Stati erano destinati a diventare sempre più dipendenti dal centro per la propria sopravvivenza finanziaria e per la determinazione della propria direzione politica.
Il 29 Luglio 1975 il regime di Yakubu Gowon fu messo da parte da un golpe militare questa volta con il plauso della popolazione. Il colpo di Stato era opera di un gruppo di militari che incanalarono la volontà della popolazione ormai stanca del regime Gowon e dell’incompiuta transizione alla democrazia. Venne insediato alla testa dello Stato il generale Murtala Mohammed, affiancato dal suo braccio destro il generale Olusegun Obansajo. Le riforme Mohammed-Obansajo portarono alla ristrutturazione del governo militare federale; all’epurazione della burocrazia, della magistratura, degli apparati parastatali, delle università; ad un ulteriore accentramento del potere federale e l’estensione del controllo statale sulla società e sull’economia.
Tra il Luglio e l’Agosto del 1979 si svolsero le elezioni per il Senato, la Camera dei rappresentanti federale, per i Parlamenti degli Stati, per i governatori dei medesimi ed infine per le presidenziali. I risultati decretarono la vittoria del National Party of Nigeria, che si affermò come partito di maggioranza relativa a livello federale. A livello presidenziale, venne eletto il candidato dell’NPN Shehu Shagari.
Anche le elezioni del 1983 sono di nuovo una storia di gravi violenze, irregolarità e frodi a tutti i livelli. Alle presidenziali Shehu Shagary venne rieletto ottenendo una larghissima maggioranza. Il paese, all’indomani delle elezioni, si trovò prossimo all’anarchia. Il 31 Dicembre 1983 l’esercito, invocato apertamente per cinque mesi da diversi settori della società, intervenne a metter fine alla Seconda Repubblica.
Il 27 Agosto 1985 dopo l'ennesimo colpo di stato, Ibrahim Babangida salì al potere e, lontano dal voler portare avanti la transizione verso la democrazia, consolidò la sua dittatura militare e arrogò a se il massimo dei poteri.
Il ritorno alla democrazia nel 1999 sotto il presidente Obansajo avvenne dopo anni di transizione che vide dopo le elezioni presidenziali annullate del 1993 l'instaurazione di un governo militare a guida del generale Abachi, segnata da anni di feroce dittatura, violenza civile e politica.
Oggi, la Nigeria rappresenta lo Stato africano più popoloso, con 152.217.341 abitanti, una densità di 96,3 persone per Km2, ed un tasso di crescita di 1,96% nel 2010. Il 41% della popolazione è al di sotto dei 15 anni, mentre solo il 3,1% della popolazione è over 65. La popolazione nigeriana al 2010 è prevalentemente di tipo rurale, mentre il 48% vive in aree urbane .
La Nigeria è un Repubblica Federale con un sistema di governo di tipo presidenziale. Presenta una struttura di governo tripartita, formata dal Governo Federale composto da 36 Stati, ai quali si aggiunge la Capitale Federale (Federal Capital Territory), che forma uno Stato a se. I 36 Stati e la FCT sono divisi amministrativamente in 774 aree di governo locale. Gli Stati sono raggruppati in sei aree geo-politiche: Nord-Est, Nord-Ovest, Centro-Nord, Sud-Ovest, Sud-Est ed infine Sud-Sud.
La Nigeria è un Paese etnicamente e religiosamente complesso, che racchiude al suo interno più di 400 gruppi etnici e linguistici, ed i cui cittadini praticano diverse religioni e provengono da background culturali diversi.
I musulmani rappresentano il 50% della popolazione, mentre i cristiani sono il 40%; il restante 10% è composto da religioni o credenze di matrice autoctona.
La Nigeria è il quinto Paese al mondo come produzione di petrolio che domina l’intera economia locale, rappresentando il 95% delle esportazioni. Nel 2009 la Nigeria ha registrato un Prodotto Interno Lordo pari a 339 miliardi USD, registrando una crescita del 6% rispetto all’anno precedente, ed un reddito procapite che si aggira attorno ai 2,300 USD. Se la Nigeria è il 33° Paese più ricco al mondo, tuttavia nella classifica del reddito procapite è situata al 184 posto. Il 74% della popolazione vive al di sotto della soglia di povertà.
Lo sviluppo in Nigeria appare molto complicato, ed è il risultato di politiche scellerate basate sul petrolio che si sono sviluppate negli anni e che sono state causa tanto dei successi quanto dei fallimenti dei numerosi regimi militari che si sono succeduti nel tempo. La Nigeria occupa la 142 posizione nel Human Development Index, piazzandosi tra i 30 Paesi più poveri al mondo .
1. 2. La Nigeria e l’HIV/AIDS
A differenze della regione Orientale e Centrale dell’Africa, dove i primi casi di HIV/AIDS furono identificati agli inizi degli anni Ottanta, solo nel 1986 venne riportato il primo caso di contagio in Nigeria. Questo caso fu diagnosticato in una prostituta ghanese che aveva praticato la sua attività in Costa d’Avorio .
Agli inizi dell’epidemia dell’HIV/AIDS, l’attenzione del mondo fu rivolta sopratutto ai Paesi dell’Africa Subsahariana quali Tanzania, Zaire, Uganda, Kenya, Malawi e altri Paesi dell’Africa Centrale e Orientale. Nell’Africa Australe l’attenzione fu rivolta a Paesi quali la Namibia, lo Zimbabwe, il Botswana e più tardi al Sudafrica. Queste aree erano viste dalla maggior parte dell’opinione pubblica internazionale come il focolaio dell’epidemia, ed in esse si registravano i maggiori casi di contagio, mentre la regione Occidentale dell’Africa era ritenuta generalmente libera e non ancora “infettata” dal virus.
Il sentimento generale agli inizi dell’epidemia vedeva proprio la Nigeria come uno dei Paesi che per ultimi furono colpiti e affetti dall’HIV/AIDS.
Il basso livello di alfabetismo, un sistema sanitario fortemente arretrato, la difficoltà di accesso alle cure da parte della popolazione povera nigeriana, assieme a pratiche mediche e igieniche legate alla tradizione, hanno fatto si che il problema dell’infezione da HIV venisse preso sottomano dalla popolazione e dal governo. Anche grazie ai regimi militari che si succedevano nel periodo in cui l’epidemia cresceva in Nigeria, l’attitudine iniziale del governo quanto della popolazione nei confronti dell’epidemia era di negarne l’esistenza .
Come Paese più popoloso dell’Africa, la Nigeria è responsabile del 20% delle persone affette da HIV/AIDS in tutta l’Africa e del 10% in tutto il mondo. Nell’arco di due decenni, l’HIV/AIDS è diventata in Nigeria una “epidemia generalizzata” e i dati attuali suggeriscono come questa sia ancora in una fase emergente, e come si sia ancora lontani dallo sconfiggerla o quantomeno contenerla .
I primi dati, risalenti al 1991 mostrano un costane incremento nella percentuale di nigeriani infetti dal virus dell’HIV. Se nel 1992, la percentuale di persone infette nella regione nigeriana era dell’1,8%, nel 1994 la percentuale è raddoppiata, per arrivare ad un picco del 5,8% nel 2001, a cui è seguito un leggero calo. Gli ultimi dati disponibili riportano che, in Nigeria, le persone che vivono con l’HIV/AIDS sono 2,98 milioni, con una percentuale del 4,6%. Nel 2009, le morti a causa dell’AIDS sono state 192.000. Uno degli impatti economici e sociali più importanti causati dall’impatto dell’HIV/AIDS è il costante incremento degli orfani da genitori affetti da AIDS, stimato nel 2009 a 2,175 milioni . Gli over 15 anni a vivere con il virus sono oltre 2,4 milioni (il 3,6%) mentre sotto i 15anni i casi si attestano attorno a 220000.
Con una media del 4,6% nel 2008, è stimato che le persone che vivono con l’HIV in Nigeria siano 2.98 milioni. È, nei numeri, la seconda più larga epidemia nel mondo dopo quella del Sudafrica, e la più grande nella regione dell’Africa occidentale. Ci sono, considerevoli variazioni tra stati e aree di governo locale. A livello regionale si registrano tassi di contagio più bassi nell’area del Sud-Ovest (2,0%) mentre il tasso più alto è nell’area Sud-Sud (7,0%). A livello statale, la percentuale varia dall’1% (come ad esempio nello Stato dell’Ekiti o del Jigawa) al 10,6% (registrato nello Stato del Benue), con una maggiore prevalenza tra le donne e le ragazze.
2. 3. L’AIDS come fattore regionale e di gruppi di età
Guardando la cartina federale della Nigeria, la prima cosa che si può notare è come tutti e i 36 Stati, a cui si aggiunge la FCT, registrano casi di HIV/AIDS. Ciò, evidentemente, significa che non ci sono zone o aree nel quale l’epidemia non si sia diffusa. In base ai dati dell’ultimo rapporto disponibile, datato 2007, del National HIV/AIDS and Reproductive Health Survey (NARHS) in Nigeria, l’epidemia è più alta tra le donne (4,0%) che tra gli uomini (3,2%); così come è più alta nelle aree urbane (3,8%) rispetto alle aree rurali (3,5%). Sia nelle aree rurali che in quelle urbane, è sempre tra le donne che si riscontrano i maggior numeri di contagio. Questi sono più alti nella fascia di età dai 30 ai 39 anni (5,6%) e più bassi nella fascia di età tra i 15 e i 19 anni (1,7%). Ogni giorno più o meno 6000 giovani tra i 15 e i 25 anni contraggono l’infezione da HIV .
Nei Paesi in via di sviluppo è stimato che circa 1/3 delle malattie sessualmente trasmissibili sia riscontrabile nelle persone di fascia d’età tra i 13 e i 20 anni. Le ricerche dimostrano come in Nigeria il primo rapporto sessuale si consumi tra i 10 e i 19 anni, ed in media a 17 anni.
Secondo il NARHS report, si riscontrano più casi di sieropositività (4,6%) tra coloro che hanno ricevuto un’istruzione fino al terzo grado di studi (medie-superiori) rispetto a coloro che invece non hanno ricevuto alcuna educazione (2,7%). Guardando la cartina, possiamo dedurre che alcune zone possano essere considerate più ad alto rischio rispetto ad altre. La ragione è ancora poco chiara, forse assente, anche perché bisogna considerare la notevole migrazione tra i diversi stati all’interno della Nigeria, dove quindi può capitare che una persona possa essersi infettata al di fuori del proprio stato natale ma in seguito costretta a rientrarvi proprio come risultato dell’essere HIV positiva.
Lo Stato dell’Ekiti nell’area Sud Ovest della Nigeria registra il livello più basso di persone che vivono con l’HIV/AIDS (1%), mentre lo Stato del Benue nella zona del Centro Nord registra il tasso più alto (10,6%). Possiamo aggiungere che l’area locale del Bwari situata nella FCT è il sito che registra una maggiore percentuale raggiungendo il 22%.
Il rapporto mostra come le donne che hanno ricevuto un educazione di primo, secondo o terzo livello, registrino tassi di HIV/AIDS rispettivamente pari al 5,1%, 5,8% e 4%. Le donne single affette dal virus sono il 5,9% contro il 4,7% delle donne sposate. Ancora, il 5,4% delle persone affette dal virus vivono nella zona Cento Nord, e si differenziano tra area urbana dove il tasso raggiunge il 6,2% e la zona rurale dove si attesta al 4%. L’area del Nord Ovest registra un tasso del 2,4%, nelle zone rurali del 2% e nelle zone urbane del 2,6%. L’area del Sud Est registra un tasso del 3,7%: nelle zone rurali del 3,4% e nelle zone urbane del 5,4%. Il Sud Ovest si attesta al 2%, e vede nelle aree rurali un tasso di prevalenza del virus del 1,7% contro il 2% delle aree urbane. Infine nell’area Sud Sud il tasso di prevalenza del virus è del 7%, di cui il 4% nelle zone rurali e il 7,1% nelle zone urbane .
Gli ultimi dati disponibili, risalenti al 2008, mostrano come in Nigeria sia la fascia d’età tra i 25-29 anni a registrare i più alti tassi di infezione, mentre le percentuali si riducono notevolmente nella fascia di età dei 40-44 anni, che risulta quella con il numero di persone infette più basso.
3. 4. L’HIV/AIDS tre le fasce a rischio della popolazione
L’HIV/AIDS Integrated Biological and Behavioural Surveillance Survey (IBBSS) è stata condotta nel 2007 tra la popolazione i cui comportamenti o lavori li esponevano a più alto rischio di contrarre malattie sessualmente trasmissibili. Questi gruppi includono le prostitute (FSW, female sex workers), gli uomini che hanno rapporti sessuali con altri uomini (MSM, men who have sex with men), gli utilizzatori di droghe endovena (IDU, injecting drug users), gli impiegati nel settore dei trasporti (TW, transport workers) e coloro che operano nelle forze armate e forze di polizia.
Nonostante i programmi di prevenzione e controllo rivolti specificatamente a questi sottogruppi, e sebbene il numero di contagi in Nigeria sia in diminuzione , l’epidemia tra questi sottogruppi è in aumento.
Si riscontra il numero di contagi maggiore tra le prostitute dei bordelli (37,4%), seguite dalle prostitute di strada (30,2%), gli MSM (13,5%), gli IDU (5,6%), i TW (3,7%), le forze di polizia (3,5%) e le forze armate (3,1%) .
In Nigeria la prostituzione è illegale, ma più di un milione di ragazze lavora in bordelli o per strada. Essendo un’attività clandestina, il commercio del sesso è effettuato sopratutto nei sobborghi delle città. Per molte prostitute questo è un lavoro temporaneo, intrapreso per racimolare abbastanza denaro per poi potersi cimentare in altre attività. Le prostitute risultano essere il sottogruppo maggiormente colpito dall’HIV/AIDS. In alcuni Stati, si può notare come le percentuali di infezione all’interno di questa categoria salgano, come nel Kano, al 49,1%. L’abuso di alcool è sovente associato. Lo stato del Lagos registra la più bassa percentuale, del 23,5% tra le prostitute da bordello, e il 12,9% tra le prostitute di strada; un dato interessante se si considera il fatto che in questo Stato si registra la più alta percentuale nell’utilizzo del preservativo (98,2% tra le prostitute da bordello, e 98,6% tra le prostitute di strada). Le ricerche mostrano inoltre come tra le prostitute l’utilizzo del preservativo sia minore con i propri partner (38,1% tra le prostitute da bordello, e 46,1% tra le prostitute di strada), mentre raggiunge livelli elevati con partner casuali (83,3% e 84,8%). Ciò significa che il rischio di contrarre l’infezione è maggiore per i partner delle prostitute piuttosto che per i loro clienti risultando così essere un potenziale ponte per la diffusione dell’epidemia tra una popolazione non racchiudibile in nessun gruppo, ovvero la popolazione generale .
Tra il gruppo rappresentato dagli uomini che hanno rapporti sessuali con altri uomini, la percentuale di contagiati si attesta al 13,5%. Questo gruppo è formato soprattutto da ragazzi sotto i 25 anni, che ne rappresentano il 75% del totale . I MSM mostrano un alto livello di conoscenza sulla prevenzione all’HIV, ma tuttavia l’uso costante del preservativo è minore rispetto alle percentuali registrate tra il gruppo formato dalle prostitute. È un gruppo al quale sono tutt’oggi rivolti pochi programmi di prevenzione. L’utilizzo non costante del preservativo può essere naturalmente un fattore di rischio per la diffusione della malattia anche tra la popolazione generale, soprattutto quando gli appartenenti a questo gruppo hanno rapporti non protetti con le proprie partner femminili.
I dati del HIV/STI Integrated Biological and Behavioural Surveillance Survey (IBBSS) del 2007 mostrano come coloro che utilizzano droghe iniettate endovena, rappresentino il terzo gruppo maggiormente colpito dall’HIV/AIDS. Anche questo sottogruppo, come gli MSM, è costituito sopratutto da più giovani, il cui 60% è sessualmente attivo. All’interno di questo gruppo, solo il 40% utilizza materiale sterilizzato durante l’iniezione della sostanza stupefacente .
Tra le fasce a rischio le categorie che risultano essere le meno colpite dall’epidemia sono i lavoratori del settore dei trasporti, le forze armate e quelle di polizia. Come mostra la tabella, le cosiddette “fasce a rischio” rappresentano invece soltanto il 31% delle nuove infezioni. Questi dati stanno a dimostrare come non ci siano in realtà fasce a rischio, ma soltanto comportamenti a rischio.
2. 5. Fattori di diffusione dell’HIV/AIDS in Nigeria
2.5.1 Fattori sociali
Nonostante l’HIV/AIDS non conosca status sociale, appartenenza etnica, razza e religione, si può tuttavia affermare che alcune persone siano più vulnerabili e facili a contrarre l’infezione che altre.
La Nigeria è stata a lungo considerata un Paese in cui l’epidemia di HIV/AIDS fosse qualcosa di marginale, che riguardasse piuttosto altre regioni e altri Stati africani, considerando a lungo se stessa come un Paese libero da questo tipo di virus e di malattia. Malgrado questo fatto, molti studiosi misero in guardia il Paese circa il pericolo di questo autocompiacimento. Piuttosto che preparare il terreno e informare le persone sui pericoli di questa epidemia, ci fu una negazione totale del problema da parte del governo e della classe intellettuale, e di conseguenza da parte della popolazione in generale.
È noto come nonostante le denunce da parte di medici di primi casi di diagnosi di sintomi simili a quelli riscontrabili nei pazienti affetti da AIDS, le Federal Health Authorities nigeriane continuassero con forza a negarne l’esistenza.
I motivi di questo comportamento restano sconosciuti. Ci fu una generale assenza di ammissione di come l’epidemia fosse reale e di come potesse facilmente essere contratta da chiunque giusto attraverso un rapporto sessuale. Questa reiterata negazione portò ad un atteggiamento passivo della maggioranza della popolazione, che pensava fosse una malattia riguardante l’Occidente. La World Health Organisation (WHO) ha definito nel 1958 la “salute” come “stato di completo benessere fisico, psichico e sociale e non semplice assenza di malattia ”. Da questa definizione, possiamo capire come la salute nell’essere umano vada oltre la mera funzione biologica dell’organismo. In tale contesto, la salute viene considerata più un mezzo che un fine e può essere definita come una risorsa di vita quotidiana che consente alle persone di condurre una vita produttiva a livello individuale, sociale ed economico. Questa definizione riconosce un collegamento tra la salute del corpo e la salute della persona, e non solo nel contesto della propria individualità, ma in rapporto all’intera comunità. Per essere in salute, una persona deve interagire col proprio ambiente e mantenere se stesso all’interno di quell’ambiente.
Partendo da questa definizione di salute, è chiaro come il disagio sociale, economico e politico della società nigeriana siano in relazione con la malattia, ed in particolare con la rapida diffusione di questa.
Dagli anni ottanta all’inizio del nuove millennio, gli ospedali nigeriani erano stati ridotti a mere cliniche di consulenza. I servizi sanitari e le misure di prevenzione di base, quali una basilare educazione sanitaria, l’erogazione di acqua pulita, cure per le madri incinta e i nascituri, scorte di medicinali, cure adeguate a malattie comuni, una adeguata nutrizione, e così via, mancavano nella maggior parte degli ospedali, e ad oggi, sebbene dei leggeri miglioramenti, rimane ancora un grave problema, sopratutto per le classi più disagiate .
Un sistema sanitario inadeguato può essere visto nel settore delle trasfusioni di sangue. È un fatto risaputo che la trasfusione di sangue infetto ha contribuito enormemente alla diffusione dell’HIV/AID. In Nigeria, la maggior parte degli ospedali non detiene i macchinari per l’analisi del sangue, a causa anche degli alti costi di una procedura come può essere quella della trasfusione. Non deve dunque sorprendere che sangue infetto possa essere stato trasfuso in persone innocenti e sopratutto insospettabili .
Collegato col problema di un sistema sanitario inadeguato vi è il problema dell’economia. Diversi studiosi hanno storicamente affermato l’importanza dell’economia a riguardo della salute, e in particolare una connessione tra la povertà e la salute, per cui l’effetto dell’arretratezza economica nei Paesi in via di sviluppo non colpisce soltanto il settore economico ma tutti i settori. Una economia povera avrà maggiori difficoltà a perseguire delle politiche sanitarie efficienti, e quindi dispendiose, così come finanziamenti alla ricerca.
Non è un caso che tra il 1992 e il 1998, periodo in cui la Nigeria attraversò un drammatico incremento delle infezioni di HIV/AIDS, la situazione generale del Paese era in uno stato di caos, che alimentò la diffusione stessa dell’infezione. In situazioni nelle quali la libertà politica è soppressa, come durante i regimi militari di Shehu Shagari e di Sani Abacha, non è difficile capire come il diritto dei cittadini ad accedere alle cure fosse in realtà un problema inesistente per la classe politica.
Nel settore della sanità, un problema importante ma spesso non considerato è la difficoltà di reperire personale medico che possa lavorare nelle poche e talvolta fatiscenti strutture ospedaliere. Uno dei fattori che rende difficile reperire questo tipo di personale è l’emigrazione di personale medico e infermieristico. Questo è dovuto al fatto che la maggior parte di dottori e infermieri provengono dalle zone urbane e dunque difficilmente accettano di lavorare nelle zone rurali.
Le zone rurali sono caratterizzate da assenza di fognature, accesso fortemente limitato all’elettricità, servizi sociali primitivi, qualità scadente delle strutture scolastiche e delle vie di comunicazione, un sistema sanitario fragile e accesso all’acqua potabile inadeguato. In tali condizioni, il numero del personale medico ed ospedaliero dovrebbe quantomeno essere raddoppiato per far si che la popolazione abbia accesso alle cure mediche di base .
Infine vi è anche il problema della capacità economica della popolazione di affrontare i costi dei servizi sanitari. Se da una parte, la costante bassa qualità ed inadeguatezza dei servizi sanitari pubblici ha reso quasi una scelta obbligata rivolgersi al settore sanitario privato, dall’altra parte bisogna considerare che oltre il 74% della popolazione vive al di sotto della soglia di povertà, ovvero con meno di 30 USD al mese, il che rende l’accesso ai servizi sanitari impossibile . Anche dove è presente l’accesso a servizi sanitari pubblici, molte persone non sono in grado di affrontare quelle spese minime. In Nigeria, ad oggi, decine di migliaia di persone continuano a morire per malattie che sono facilmente curabili.
È chiaro che il sistema sanitario nigeriano è tanto fragile quanto inadeguato. Nessun Paese può mantenere una crescita economia stabile in assenza di un adeguato sistema sanitario. Il problema della sanità è un enigma in Nigeria, ed i suoi cittadini sono i primi a soffrirne le conseguenze. Risolvere il problema richiede un approccio aggressivo da parte del Governo federale, degli Stati e dei governi locali, che devono lavorare in maniera coordinata. Il problema della sanità è un’emergenza nazionale e come tale deve essere considerata dal Governo in tutte le sue ramificazioni. Con la devastazione provocata dall’HIV/AIDS e da altre malattie prevenibili, la Nigeria deve mettersi al lavoro e portare avanti programmi di sanità pubblica per evitare future calamità .
Per stigmatizzazione si intende quel fenomeno sociale che attribuisce una connotazione negativa ad un membro o ad un gruppo della comunità, in modo da declassarlo ad un livello inferiore . Questo fenomeno ha certamente contribuito alla diffusione dell’epidemia in Nigeria così come in altre parti del mondo. La stigmatizzazione si basa su un pregiudizio che fa si che chiunque sia portatore del virus sia condannato sotto diversi punti di vista. Quando una persona viene identificata come HIV positiva, si pongono in maniera automatica da parte della comunità domande riguardo i comportamenti sessuali di quella persona, che si tendono dunque a condannare. A differenza di malattie quali il diabete, il cancro, o malattie cardiovascolari, l’AIDS porta con se lo stigma della malattia. Ciò è spiegabile col fatto che nella maggior parte dei casi, le malattie citate sono ereditarie e non acquisite dal paziente tramite azioni dirette come nel caso dell’AIDS.
Il pericolo della stigmatizzazione è doppio in quanto porta un numero elevato di persone, che probabilmente sono a rischio di contagio, a non voler effettuare il test per la paura di risultare positivi, subendo quindi le conseguenze sociali, economiche e familiari dell’essere portatore della malattia. In questo modo, si crea un circolo vizioso in quanto persone potenzialmente affette dal virus potrebbero mettere altre persone al rischio di contagio attraverso comportamenti dannosi verso se stessi e verso gli altri, il più comune dei quali è avere rapporti sessuali non protetti.
La stigmatizzazione è fortemente intrecciata con le costruzioni culturali. Spesso, alcune malattie vengono tacciate come frutto di stregoneria. L’AIDS non è in questo caso una eccezione.
Urbanizzazione e migrazione della popolazione sono stati fattori che hanno contribuito alla diffusione dell’HIV/AIDS in Nigeria. Migranti erano, e sono, sopratutto gli uomini, talvolta sposati, che lasciano le mogli nelle aree rurali e cercano lavoro nelle aree urbane. Il risultato è stato che spesso questi uomini intrattenevano delle relazioni con altre donne nelle località dove svolgevano l’attività lavorativa, mentre altri cercavano intrattenimento attraverso lo sfruttamento del mercato della prostituzione. Una volta tornati a casa, nelle aree rurali di appartenenza, era facile dunque diffondere il virus tra le proprie mogli.
Anche se, come abbiamo notato, il rapporto tra i contagi nelle zone rurali e in quelle urbane è piuttosto bilanciato, è ancora fortemente radicata nella popolazione l’idea che il maggior numero delle infezioni sia localizzata nelle aree urbane.
C’è da aggiungere, che non tutte le persone che migrano nelle aree urbane in carca di lavoro hanno la fortuna di trovarne uno. Ciò è vero sopratutto per le donne, che sono dunque “costrette” alla prostituzione come unica via di sostentamento. Un lavoro che è sicuramente maggiormente lucrativo nelle città e nelle aree urbane ad alta densità di popolazione, ma che allo stesso tempo facilita la veloce diffusione della malattia.
2.5.2. Fattori Economici
Come è facile intuire, l’economia gioca un ruolo centrale nella diffusione dell’HIV/AIDS, tanto nell’intero pianeta, quanto più nei Paesi in via di sviluppo. La malattia sembra essere concentrata presso la popolazione che è già marginalizzata sia socialmente che economicamente.
L’economia nigeriana ha attraversato un costante deterioramento nelle ultime tre decadi, dove ha registrato decrementi o leggeri incrementi del proprio prodotto interno lordo, ed un enorme debito pubblico che paralizza qualsiasi possibilità di crescita.
L’effetto più eclatante delle difficoltà economiche del Paese, e della cattiva gestione di queste, è che nonostante una crescita economica attestata nel 2009 al 6,1%, un PIL di 173,3 USD miliardi, che rende la Nigeria il 33° Paese più ricco del mondo, la popolazione che vive al di sotto della soglia di povertà, con meno di 1,30 USD al giorno, è del 74%.
La diffusa povertà non riguarda soltanto le classi “rurali” e svantaggiate, ma ha colpito anche quella che un tempo era considerata la classe media. La società nigeriana è divisa tra classe estremamente ricca e classe povera, con una quasi inesistente classe media. Vi è una enorme disparità tra le due classi. A causa di questa disparità, possiamo notare una forte sperequazione delle risorse. La stragrande maggioranza della popolazione manca di accesso ai più basici mezzi di sostentamento, come la buona nutrizione, l’accesso all’acqua potabile, alloggi, servizi scolastici e sanitari. La povertà è talmente forte che molte di queste persone non possono nemmeno permettersi di pagare le visite mediche. Di conseguenza, non si recano neppure negli ospedali. Molte famiglie sono così costrette a mandare i propri figli, specialmente le ragazze più giovani, a lavorare per la strada, nei bar e negli hotel, par far si di portare ulteriori, se non uniche risorse necessarie al sostentamento della famiglia.
Questa situazione si venne a creare nel periodo della dittatura del Generale Sani Abacha. Fu in questo frangente della storia nigeriana che l’economia andò in pezzi, ed il Paese perse la propria immagine a livello internazionale. E fu proprio in questo periodo che la Nigeria conobbe un feroce incremento delle infezioni da HIV. Non è dunque un caso che HIV/AIDS e povertà siano strettamente correlati .
Il costo dei trattamenti per le persone infette, ha certamente contribuito alla diffusione della malattia. In passato, la mancanza dei farmaci o comunque il loro costo esorbitante, è stato un incubo per le persone che vivevano con l’HIV/AIDS, nonostante gli sforzi da parte del governo di rendere i trattamenti più accessibili.
Alcuni dati risalenti al 2003, possono aiutare a rendere più chiara l’idea dell’impossibilità di accesso ai trattamenti da parte della maggioranza della popolazione. La terapia antiretrovirale, a quell’epoca, aveva un costo approssimativo di 10.000 USD ad anno per paziente. Considerando che lo stipendio medio di un nigeriano si aggira attorno ai 660 USD all’anno, possiamo capire l’impossibilità d’accesso alle cure. Risulta chiaro come un paziente affetto da HIV/AIDS non possa affrontare le spese necessarie all’acquisto dei farmaci, in quanto dovrà comunque pensare a bisogni primari quali il cibo e altri beni di prima necessità. Ciò significa che fino a pochi anni fa non vi era speranza per le persone sieropositive in questo Paese. Oggi la Nigeria ha fatto grandi passi avanti nell’accesso alle cure mediche per pazienti affetti da HIV/AIDS, iniziando a produrre i medicinali nelle stesse case farmaceutiche nigeriane, che porterà ad una riduzione dei costi da 240.000 naira a 60.000 naira all’anno (400 USD circa) .
Una delle conseguenze della situazione economica disagiata della Nigeria è la perdita di posti di lavoro, specialmente nel settore del lavoro non qualificato, che ha portato molte imprese e compagnie a tagliare il numero dei propri operai. La conseguenza è stata una esponenziale crescita del numero dei disoccupati nel Paese (il tasso di disoccupazione si attesta intorno al 28%). Legato al problema della disoccupazione vi è il crescente fenomeno della vendita ambulante, che è diventato una delle maggiori caratteristiche nella maggior parte delle città nigeriane. Questa pratica è comune nelle fermate del bus, nei parcheggi, nella maggiori aree di sosta stradali, ai semafori e nel traffico. Questi venditori ambulanti sono specialmente giovani ragazze, molte delle quali hanno tuttavia se non un educazione media-superiore, quantomeno elementare. Molte di loro hanno dovuto abbandonare gli studi a causa della povertà o per altre ragioni. Questi venditori ambulanti contribuiscono alla diffusione dell’HIV/AIDS in quanto spesso queste ragazze non vendono solo le loro merci, ma anche i loro corpi, per incrementare i bassi guadagni delle vendite .
La prostituzione gioca un ruolo fondamentale e di primo piano nella diffusione dell’HIV/AIDS in Nigeria così come in generale in tutta l’Africa. La prostituzione è legata naturalmente alla povertà.
Il mercato della prostituzione è forte sopratutto nei centri abitati e nelle città più grandi della Nigeria. Ciò è dovuto al fatto che nelle città vi sono, oltre ad un numero maggiore di persone e quindi di potenziali clienti, strutture adatte quali grandi e piccoli hotel, bar, nightclub, dove le giovani nigeriane possono offrire sesso in cambio di soldi.
2.5.3. Fattori politici
Così come i fattori sociali ed economici, i fattori politici hanno e continuano a giocare un ruolo significante nella diffusione dell’HIV/AIDS in Nigeria.
Come già accennato in precedenza, vi fu negli anni Ottanta un riluttanza da parte del governo ad ammettere la presenza dell’HIV/AIDS nel Paese. Di conseguenza, la popolazione era completamente impreparata e disinformata sui danni di quella che era una epidemia già da tempo presente nel Paese e che rapidamente si stava diffondendo tra la popolazione inconsapevole. Bisognerà aspettare il 23 Agosto del 1991 perché la prima campagna ufficiale contro l’HIV/AIDS venga lanciata dal Governo Federale.
L’HIV/AIDS fu vista soltanto come un problema di salute pubblica, e mai attraverso un approccio multisettoriale. Un approccio di questo tipo avrebbe significato che diversi ministeri, a partire da quello della Sanità, si sarebbe attivato nella Campagna contro l’HIV/AIDS; avrebbe inoltre significato un coordinamento tra i diversi settori del governo ad ogni libello, incluse le Organizzazioni non governative così come la comunità accademica.
Sin dall’indipendenza dall’Inghilterra, dichiarata il 1 Ottobre del 1960, i militari hanno dominato la vita politica della nazione. I militari hanno governato dal 1966 al 1979 e poi dal 1984 al 1999. All’autoritarismo seguì lo sfruttamento da parte della classe ricca della classe povera ed emarginata. Inoltre, i regimi militari creano instabilità e sottosviluppo.
Storicamente i regimi militari e le regole da questi imposte, hanno causato devastazione per l’intero Paese. Queste devastazioni, che hanno implicato il campo sociale e quello economico, hanno contribuito notevolmente alla diffusione dell’HIV/AIDS. Esempi degli effetti negativi della dittatura militare sono facili da trovare. Non è un caso che il settore della sanità pubblica iniziò a deteriorarsi durante la dittatura del Generale Ibrahim Babangida, che fu a capo del regime militare dall’agosto 1985 all’agosto del 1993. Fu in questi anni che ad esempio si registrò una notevole emigrazione di dottori e infermieri dalla Nigeria all’Arabia Saudita e altre parti del mondo, in cerca di possibilità lavorative . Questo esodo di massa rappresentò la mancanza di soddisfazione nel lavoro causato dalle misure e dalle politiche intraprese dall’amministrazione Babangida. Il sistema sanitario pubblico iniziò a deteriorarsi per la “fuga di cervelli” e molti di quegli ospedali che nel passato era efficienti e famosi per le cure date diventarono delle mere “cliniche consultive”, a causa anche dell’indisponibilità di farmaci e della mancanza di incentivi per la ricerca.
Il successore di Babangida, il generale Sani Abacha, che fu a capo del regime militare dal Novembre 1993 al Giugno del 1998, non fece altro che peggiorare una situazione già di per se disastrosa. Il suo Governo fu segnato da una mancanza di stabilità politica, da tirannia e oppressione in ogni angolo del Paese.
I mass media, che potevano giocare un ruolo importante nell’informazione a favore della battaglia contro la minaccia dell’HIV/AIDS, furono costantemente tormentati, sotto attacco e sotto sorveglianza. Molti giornalisti scomparvero misteriosamente mentre un gran numero fu imprigionato o condannato ai lavori forzati. Come risultato dell’oppressione e dell’autoritarismo molte persone cercarono nel piacere sessuale una via di fuga dalla realtà del Paese. Il risultato fu il numero allarmante di persone che furono contagiate dal virus . L’incursione dei militari nella vita politica del Paese creò instabilità che si rifletteva nel significante aumento di contagi da HIV/AIDS .
La corruzione è da sempre uno dei fattori che caratterizzano tutte le amministrazioni che si sono succedute in Nigeria. Ad oggi, secondo la classifica dell’organizzazione “Transparency International”, la Nigeria si trova al 134° posto dell’indice di corruzione percepita . In altri termini, la Nigeria è un Paese nel quale la corruzione è dilagante, ed è diventata un imbarazzante fenomeno che viene associato costantemente al Paese. La corruzione è visibile ovunque, tra i leader del Paese così come tra i banchi di scuola. Gli effetti sono rintracciabili in un debito pubblico enorme, infrastrutture fatiscenti in ogni settore e specialmente in quello della sanità ed il fatto che la maggioranza della popolazione rimane impantanata nella povertà più assoluta.
La corruzione a tutti i suoi livelli contribuisce alla diffusione dell’HIV/AIDS nella misura in cui quelle risorse potrebbero essere usate per alleviare la sofferenza della persone. La corruzione in Nigeria ha raggiunto livelli tali che anche le risorse allocate per la battaglia all’HIV/AIDS attraverso il Petroleum Trust Fund (PTF) furono presumibilmente gestite male . L’acquisto di kits per le analisi del sangue comprate attraverso il PTF nel Gennaio del 1999 furono da subito criticate. Il PTF annunciò alla prima conferenza nazionale sull’AIDS tenutasi ad Abuja nel 1999 che si sarebbero distribuiti kits per le analisi del sangue e per diagnosticare l’HIV in ogni ospedale e in altre strutture sanitarie secondarie. Ma i medici e il personale che giornalmente lavorava sul campo argomentarono che l’enorme spesa affrontata per dotare le strutture sanitarie di questi macchinari era insensata. Fondi per campagne di prevenzione presso le fasce della società più esposte e per i medicamenti, erano in quel momento storico più importanti se non fondamentali. Le critiche erano ancora più forti in quanto all’epoca il PTF rappresentava l’unica agenzia autoctona che disponeva di fondi per la lotta all’HIV/AIDS, che puntualmente venivano utilizzati in maniera poco opportuna se non addirittura sprecati .
Alla luce di questi fatti non è sorprendente che programmi designati al controllo dell’epidemia non possano essere portati a termine a causa della mancanza di fondi. Ancora, nella maggior parte degli ospedali pubblici, medicine e altre apparecchiature mediche necessarie al funzionamento di queste strutture vengono sottratte dal personale medico o amministrativo e utilizzate per loro progetti privati o nelle loro cliniche private.
Uno dei fattori che ha contribuito alla diffusione del virus ma che troppo spesso viene considerato in maniera superficiale è la guerra e le violenze in generale. Dalla fine della guerra civile nigeriana nei primi anni Settanta, la Nigeria non è stata più attraversata da nessun’altra guerra, anche se sporadicamente vi sono stati conflitti ed episodi di violenza, sopratutto negli ultimi anni del XX secolo. Oltre alla perdita della propria vita e della proprietà, molte donne e giovani nigeriane hanno subito un elevato numero di stupri. Sfortunatamente questi casi di stupro vennero da sempre nascosti e non denunciati. In una situazione del genere è facile diffondere la malattia. Inoltre, i soldati nigeriani impegnati in operazioni di peacekeeping in Liberia tra il 1997 e il 1998, sono stati accusati di aver perpetuato abusi indiscriminati tra le donne liberiane, lasciando nel Paese più di 25.000 bambini, abbandonati dopo la fine della missione. Stesse accuse furono rivolte per i soldati nigeriani impegnati in missione di peacekeeping in Sierra Leone. È facile intuire come questi soldati abbiamo acquisito e diffuso il virus sia durante il periodo di guerra che nel Paese una volta rientrati .
Lo stile di vita in tempo di guerra (cosi come nelle operazioni di peacekeeping) è caratterizzato da un alto livello di partner sessuali, scarso utilizzo del preservativo, ed esposizione alle trasfusioni di sangue. In Nigeria agli inizi era tenuta nascosta l’esistenza del problema dell’HIV/AIDS tra le fila dell’esercito. Soltanto l’ammissione da parte di alcuni soldati affetti dall’HIV/AIDS ha innalzato il livello di consapevolezza dell’impatto prodotto dalle operazioni di peacekeeping nella diffusione dell’epidemia .
2.5 4. Fattori culturali
Non vi è dubbio che alcune pratiche culturali siano dannose e potenzialmente complici della diffusione dell’HIV/AIDS nel Paese. Non è un caso che ad oggi siano maggiormente le donne a vivere con l’HIV/AIDS rispetto agli uomini. Delle 22,4 milioni di persone che al 2008 vivevano in Africa Subsahariana con l’HIV/AIDS, il 57% era di genere femminile. Ad oggi, in Nigeria, dei 3 milioni di morti a causa dell’AIDS, 1,61 milioni erano donne. Nel 2009, le donne morte a causa dell’AIDS erano 106.000, contro gli 88.000 uomini .
La spiegazione è che ad oggi, le donne continuano ad essere guardate come esseri inferiori rispetto agli uomini. Nella maggior parte dei casi, nella tradizione culturale africana, sono sempre le donne ad essere le vittime. Non è un caso che se marito e moglie riescono a tenere segreto che uno o entrambi sono affetti da HIV/AIDS, il matrimonio continua a durare, ma se invece trapela la notizia che la donna sia affetta allora lei sarà la sola ad essere marginalizzata se non cacciata. Ciò non accade quando la notizia riguarda il marito. È presente nella cultura nigeriana l’idea di considerare sempre la donna come responsabile della diffusione del virus all’interno della famiglia.
L’inferiorità presunta della donna si esprime in pratiche culturali come la mutilazione genitale femminile, che mette a rischio di esposizione al virus le donne e le giovani africane, oltre che ad essere una pratica disumana. Ad esempio è stato riportato che l’uso delle stesse lame e coltelli utilizzati per tagliare le aree genitali delle giovani nigeriane nella terapia tradizionale della circoncisione femminile sia una possibile via di infezione. Attraverso questa pratica e con il contatto del sangue, è facile essere infettati e infettare. Queste pratiche non solo aiutano la diffusione dell’HIV/AIDS ma sono portatrici di infezioni dell’apparato riproduttivo, infertilità e qualche volta la morte. Circa il 60% delle donne nigeriane ha subito questa pratica, che è più comune nel Sud del Paese, dove la percentuale sale all’85%. Questa pratica viene considerata come un rituale di anzianità, praticato sopratutto in bambine tra il quarto e l’ottavo anno di età. È praticata da tutte le religioni, anche quelle che ufficialmente non la prevedono .
Un’altra pratica culturale potenzialmente dannosa presente nella cultura africana, è il mito riguardante la cura di malattie sessualmente trasmissibili, secondo cui se un uomo con una malattia venerea ha un rapporto sessuale con una vergine, allora la malattia guarirà. Il risultato è che, lontano dall’essere curata, la malattia viene invece trasmessa e diffusa. Questo mito, oltre ad essere inutile e dannoso, è oltretutto discriminate verso il genere femminile, che ancora una volta diviene vittima di una pratica culturale sessista.
Sempre su questa strada, possiamo citare la pratica culturale della poligamia, ovvero quando un uomo ha più di una sola moglie nello stesso periodo di tempo. Questa pratica si trova in molte differenti culture della multietnica società nigeriana, ed ha l’approvazione di molte religioni, tra le quali l’Islam. Spesso si argomenta che molte persone praticano la poligamia per ragioni economiche. In una società dove il 70% della forza lavoro è impiegata nel sottore agricolo, che nel passato era il settore dominante dell’economia, si credeva che avere famiglie allargate avrebbe portato abbastanza braccia da essere impiegate nella coltivazione e facilitato così la coltivazione dei beni necessari al sostentamento della famiglia ed a produrre eccedenze da immettere nel mercato. Oggi che il Paese è incentrato maggiormente sull’industria e sull’esportazione di idrocarburi, e con un’economia disastrata, è difficile sostenere una famiglia allargata, e col problema dell’HIV/AIDS, questa pratica culturale non ha che l’effetto di contribuire alla rapida diffusione dell’epidemia .
In Nigeria, non vi sono leggi che impongano un età minima per il matrimonio. I matrimoni che coinvolgono adolescenti sono ancora la norma in numerose zone del Paese. I genitori vedono in ciò un modo per proteggere le proprie figlie dai pericoli del mondo esterno e mantenere preservata la loro castità fino al matrimonio. Molte ragazze si sposano ad una età giovanissima ed è solito notare un profondo gap di età tra il marito e la giovane moglie. Anche questa pratica contribuisce alla diffusione del virus tra le donne e le giovani ragazze, in quanto i mariti, come appena detto, spesso hanno altre mogli e altre partner sessuali.
