La distrofia muscular de Duchenne (DMD) es una enfermedad genética neurodegenerativa, caracterizada por debilidad muscular progresiva. Afecta principalmente a hombres, con una incidencia de 1 de cada 3600-6000 varones nacidos vivos. Presenta una herencia ligada al cromosoma X de forma recesiva y obedece a la mutación del gen encargado de la síntesis de distrofina, proteína esencial del tejido neuromuscular. La deficiencia de acil-CoA deshidrogenasa de cadenas medias, es un defecto en la oxidación de ácidos grasos (DOAG) a nivel mitocondrial. Afecta tanto hombres como mujeres, con una incidencia de 1 de cada 4900 - 17000 individuos. Su herencia es autosómica recesiva, con una presentación clínica variable, desde sintomatología leve hasta la muerte súbita. Se caracteriza por la presencia de hipoglicemias no cetósicas en los periodos de ayuno.
Las enfermedades monogénicas, como las mencionadas, resultan de alteraciones en la secuencia de ADN de un solo gen; son condiciones relativamente poco frecuentes, que afectan a un porcentaje muy pequeño de la población mundial. La presencia de dos enfermedades monogénicas, que involucran genes diferentes, con patrones de herencia mendeliana diversa, convierte a nuestro paciente en un caso único.
PRESENTACIÓN DE CASO CLÍNICO
Paciente masculino que a los 15 días de edad fue referido por el Programa Nacional de Tamizaje, debido a elevación de los metabolitos C6 y C8 en el perfil de acilcarnitinas de la prueba de talón; fue valorado por el Servicio de Genética y Metabolismo del Hospital de Niños, donde se realizaron estudios por la sospecha de un defecto de oxidación de ácidos grasos (DOAG).
Hijo de padres consanguíneos. Sin antecedentes perinatales de importancia, con un examen físico normal. No se documentó acidosis metabólica, hipoglicemia o cetosis. En el análisis de ácidos orgánicos en orina, presentó importante elevación de hexanoilglicina, elevación moderada de ácido subérico y de suberilglicina, así como ligera elevación de ácido 7-OH-caprilico y ácido 5-OH-caproico, relacionados con la deficiencia de acilCoA deshidrogenasa de cadenas medias. El análisis molecular del gen ACADM confirmó el diagnóstico al detectarse la mutación A985G en estado homocigoto.
A la edad de 4 años, el paciente presentaba fatiga, mialgias y pérdida de las habilidades motoras previamente adquiridas. Se documentó bajo peso y talla, debilidad muscular, limitación para deambular, signo de Gowers positivo, pseudohipertrofia de gastronemios y cifoescoliosis tóracolumbar.
Se documentó elevación de transaminasas, CPK en 23439 UI/L, DHL en 1701 UI/L. Cariotipo: 46, XY. En la electromiografía presentó un patrón miopático. En el estudio por mutaciones asociadas con distrofinopatías, mediante la técnica de MLPA, se observó una deleción en el gen DMD, que abarcaba los exones del 45 al 50, lo cual confirmó el diagnóstico de distrofia muscular de Duchenne (DMD).
El paciente se encuentra en tratamiento con esteroides, mantiene un plan nutricional adecuado, asiste a fisioterapia y a la escuela de enseñanza especial. Cabe mencionar que tiene 2 medios hermanos varones, gemelos. Ambos, presentaron deleciones en los exones del 45 al 50, por lo cual también padecen de DMD. En ninguno se detectó la deficiencia de acilCoA deshidrogenasa de cadenas medias. La madre de estos niños es portadora para la deleción de los exones 45-50 en el gen DMD, y para la mutación A985G en el gen ACADM. (Figura 1)
DISCUSIÓN
La DMD es una enfermedad neuromuscular hereditaria, caracterizada por debilidad muscular progresiva. Presenta una herencia ligada al X de forma recesiva, por lo cual afecta predominantemente a hombres. Solo un 10% de las mujeres portadoras tiene alguna manifestación clínica, ya que las sintomáticas son el resultado de la inactivación del cromosoma X sano.
A nivel mundial afecta a 1 de cada 3600-6000 varones nacidos vivos. Sin embargo, en Costa Rica no se cuenta con datos sobre la incidencia o frecuencia génica de esta enfermedad. Otros países, como Canadá, han reportado una incidencia de 1 en 4700 varones nacidos vivos; en Inglaterra, de 1 de cada 18450 varones se ve afectado por esta enfermedad, y en Noruega, se presenta en 1 de cada 3917 varones nacidos vivos.
La DMD obedece a la mutación de un gen encargado de la producción de la proteína distrofina. Este gen es uno de los más complejos identificados hasta la fecha y se encuentra en el cromosoma Xp21.1. La mutación en este paciente se debe a
una deleción de tipo frame-shift, en donde se altera el marco de lectura de la proteína y se genera un codón de paro prematuro, propiciando un defecto en la síntesis de distrofina.
La distrofina se encuentra en el tejido muscular cardiaco, esquelético y liso, y en pocas cantidades en el sistema nervioso central y periférico. Esta proteína es parte importante de las membranas plasmáticas y de las uniones miotendinosas y neuromusculares.
La distrofina es una proteína del citoesqueleto, la cual se une a la actina e interactúa con un complejo glicoproteína-distrofina (CGD), que une citoesqueleto, lámina basal y membrana plasmática. En la biopsia de músculo de un paciente con DMD, la distrofina está ausente del musculo esquelético, y las fibras musculares se observan necróticas o degeneradas. Las manifestaciones clínicas en dichos individuos, son consideradas el resultado de un desbalance entre la necrosis de las fibras musculares y la regeneración mioblástica.
El diagnóstico se realiza al observar un deterioro progresivo y simétrico de la fuerza muscular proximal, limitación para deambular, marcha de puntillas, signo de Gower positivo e hipertrofia de pantorrillas. Los problemas de la columna vertebral, como la escoliosis y cifosis, pueden limitar la mecánica pulmonar y producir afección del musculo cardiaco. Generalmente, estos pacientes fallecen debido a complicaciones respiratorias o cardiacas.
La otra condición identificada en el paciente, es un defecto en la enzima acil-CoA deshidrogenasa de cadenas medias, una de las encargadas de la betaoxidación de ácidos grasos a nivel mitocondrial.
Los ácidos grasos son utilizados por el músculo cardiaco y esquelético, como principal fuente de energía durante el ejercicio, y durante el ayuno prolongado, por la mayoría de los tejidos.
La deficiencia de acil-CoA deshidrogenasa de cadenas medias, se observa en individuos de ascendencia europea. Se estima que la frecuencia mundial se encuentra entre 1:4900 y 1:17000, la variabilidad en dichas cifras se relaciona con el origen étnico de la población estudiada. El número de recién nacidos con la deficiencia enzimática detectados mediante programas de tamizaje neonatal, es superior a la esperada con base en la frecuencia de la población. Para Costa Rica, se estima, según datos no publicados del Programa Nacional de Tamizaje, que la frecuencia de aparición es de 1:25000, mucho menor que la reportada a nivel mundial.
La clínica es muy variable, desde síntomas leves, como somnolencia y fatiga, hasta vómitos, coma, convulsiones, rabdomiolisis, arritmias, cardiomiopatía, hepatomegalia, disfunción hepática y una clínica similar al Síndrome de Reye.
Los hallazgos de laboratorio son: hipoglicemia no cetósica, elevación de transaminasas, hiperamonemia y acidosis láctica. Con frecuencia las crisis metabólicas son desencadenadas por periodos de ayuno prolongado, estrés, cirugías o infecciones.
Se estima que aproximadamente un 1-3% de las muertes súbitas neonatales inexplicadas, obedecerían a un DOAG no diagnosticado de forma temprana. Sin embargo, con su incorporación en el Programa Nacional de Tamizaje en el 2006, la detección por medio de la espectrometría de masas en tándem, es fácil y rápida. En Costa Rica no se han documentado muertes súbitas neonatales relacionadas con un cribado positivo por DOAG.
El diagnóstico definitivo se realiza al interpretar los análisis de las pruebas del perfil de acilcarnitinas y ácidos orgánicos en orina, en conjunto con los estudios moleculares. Existe la posibilidad de realizar estudios prenatales en embarazos de riesgo, y cuando se conoce la mutación en uno de los padres o hermanos.
Los DOAG presentan una herencia autosómica recesiva, por lo cual los padres son portadores de la enfermedad, con un riesgo de recurrencia del 25%. Debido a que existe la posibilidad de familiares asintomáticos con la deficiencia enzimática, es recomendable efectuar análisis bioquímicos y moleculares.
En cuanto al tratamiento de nuestro paciente, aun no existen medicamentos específicos para la DMD, pero las intervenciones médicas buscan disminuir las complicaciones y mejorar la calidad de vida. Los corticoesteroides presentan un potente efecto antinflamatorio, prolongan la deambulación al preservar la fuerza, disminuyen la progresión de la escoliosis y retardan el deterioro de la función cardiopulmonar.
El tratamiento debe ser valorado individualmente, tomando en consideración los efectos secundarios de los esteroides y la presencia de condiciones prexistentes en cada persona. Hay efectos adversos comunes a largo plazo, con la administración de corticoides, sobre todo en niños y en nuestro paciente en particular, por presentar otra enfermedad concomitante. Para un manejo exitoso, se debe valorar con regularidad el crecimiento, los niveles de glicemia y la presión arterial; es preciso reducir la dosis o cambiar el régimen de administración, en caso de que los efectos secundarios sean inmanejables.
Además, en este caso se debe proveer un control nutricional estricto, cuantificando la ingesta calórica y regulando los tiempos de alimentación, lo mismo que evitar los periodos de ayuno prolongado, con el fin de prevenir los episodios de hipoglicemias.
Como se mencionó, la presencia de estas dos enfermedades genotípicamente diferentes en un mismo individuo, lo convierten en un caso muy particular e interesante, ya que no se ha descrito ningún paciente con distrofia muscular de Duchenne y DOAG en la bibliografía, según la revisión realizada para este artículo.
Asimismo, los hallazgos clínicos de fatiga, debilidad muscular y daño cardiaco, pueden ser observados en ambas condiciones, pero con tratamientos médicos diferentes, que los convierten en todo un reto médico para lograr el equilibrio necesario en su salud.
Translation - English Duchenne Muscular Dystrophy and Fatty Acid Oxidation Disorder in a Pediatric Patient
Duchenne muscular dystrophy (DMD) is an inherited neurodegenerative disorder, characterized by progressive muscle weakness. It mainly affects males, with an incidence of one in every 3,600-6,000 live male births. It is inherited in an X-linked recessive pattern and is caused by mutations in the gene responsible for the synthesis of dystrophin, an essential protein within neuromuscular tissue. Medium-chain acyl-CoA dehydrogenase (MCAD) deficiency is a fatty acid oxidation disorder (FOAD) that occurs within the mitochondria. MCAD deficiency affects both men and women, with an incidence of one in every 4,900-17,000 individuals. It is an autosomal recessive inherited disorder with variable clinical presentations, ranging from mild symptoms to sudden death. MCAD deficiency is characterized by the presence of nonketotic hypoglycemia during periods of fasting.
Monogenic diseases, like those mentioned, arise from variations in the DNA sequence of a single gene; they are relatively rare conditions that affect a very small percentage of the world’s population. The presence of two monogenic diseases, involving different genes with different Mendelian inheritance patterns, makes our patient a unique case.
CASE PRESENTATION
A 15-day old male infant was referred by the National Screening Program because of elevated levels of C6 and C8 metabolites in the acylcarnitine profile of the heel-stick test. He was assessed by Genetics and Metabolism Services at the Hospital de Niños, where studies were conducted in response to a suspected fatty acid oxidation disorder (FOAD).
The patient, born to consanguineous parents, had an unremarkable perinatal history and a normal physical examination. No metabolic acidosis, hypoglycemia or ketosis were documented. Urinary organic acid analysis revealed important elevation of hexanoylglycine, moderlately elevated suberic acid and suberylglycine, and slightly elevated 7-OH-caprylic and 5-OH-caproic acids, elevations related to the medium-chain acyl-CoA dehydrogenase deficiency. ACADM gene analysis detected the A985G mutation in the homozygous state, thus confirming the diagnosis.
When he was four-years-old, the patient presented with fatigue, myalgia, and loss of previously acquired motor skills. He was underweight and below-average height and exhibited muscle weakness, limited ambulation, positive Gowers’ sign, gastrocnemius pseudohypertrophy, and thoracolumbar kyphoscoliosis.
Increased levels of transaminase, CPK (23439 IU/L) and LDH (1701 IU/L) were documented. Karyotype was 46, XY. Electromyography revealed a myopathic pattern. Using MLPA to study the mutations associated with dystrophinopathy, we observed the deletion of exons 45-50 in the DMD gene, which confirmed the diagnosis of Duchenne muscular dystrophy (DMD).
The patient is being treated with steroids, eats a nutritionally adequate diet, receives physiotherapy and attends a special needs school. It should be mentioned that he has two half-brothers, who are identical twins. Both, having presented with deletions of exons 45-50, also suffer from DMD. Neither sibling has been diagnosed with medium-chain acyl-CoA dehydrogenase deficiency. The mother of these children is the carrier of the deletion of exons 45-50 in the DMD gene, and of the A985G mutation in the ACADM gene. (Figure 1)
DISCUSSION
DMD is an inherited neuromuscular disease, characterized by progressive muscle weakness. It has an X-linked recessive inheritance pattern, which predominantly affects men. Only 10% of female carriers have any clinical manifestation, because symptoms result from inactivation of the normal X chromosome.
DMD affects 1 in every 3,600-6,000 live male births worldwide. However, data regarding the genetic incidence or frequency of this disease are not available in Costa Rica. Other countries, like Canada, have reported an incidence of one in every 4,700 live male births; in England, one in every 18,450 males are affected by the disease, and in Norway, it presents in one of every 3,917 live male births.
DMD arises from a mutation in a gene responsible for the production of the protein dystrophin. This is one of the most complex genes identified to date and is located on chromosome Xp21.1. This patient’s mutation is due to a frame-shift deletion, in which the protein’s reading frame is altered and a premature stop codon is generated, contributing to a defect in dystrophin synthesis.
Dystrophin is found in cardiac, skeletal and smooth muscle tissues, and in small quantities in the central and peripheral nervous systems. This protein is an important part of plasma membranes as well as myotendon and neuromuscular junctions.
Dystrophin is a cytoskeletal protein, that binds to actin and interacts with a dystrophin glycoprotein complex (DGC), that connects the cytoskeleton, basal lamina and plasma membrane. In the muscle biopsy of a patient with DMD, dystrophin is absent from the skeletal muscle, and muscle fibers appear necrotic and degenerated. The clinical manifestations in said individuals are considered the result of an imbalance between muscle fiber necrosis and myoblast regeneration.
The diagnosis is made based on the observation of progressive, symmetric deterioration of proximal muscle strength, limited ambulation, tiptoe walking, positive Gower sign and calf hypertrophy. Problems with the spinal column, such as scoliosis and kyphosis, can limit respiratory mechanics and produce cardiac muscle dysfunction. Generally, these patients die from respiratory or cardiac complications.
The other condition identified in the patient is a deficiency of medium-chain acyl-CoA dehydrogenase, one of the enzymes responsible for mitochondrial beta-oxidation of fatty acids.
Fatty acids are used as the primary energy source of the cardiac and skeletal muscles during exercise, and by most tissues during periods of fasting.
Medium-chain acyl-CoA dehydrogenase deficiency is prevalent in individuals of European descent. The overall frequency of the disorder has been estimated to range between 1:49,00 and 1:17,000; the variability of this figure is related to the ethnic background of the population studied. The number of newborns detected with the enzyme deficiency by newborn screening programs exceeds expectations based on the population frequency. According to unpublished data from the Programa Nacional de Tamizaje (National Screening Program), it is estimated that the incidence in Costa Rica is 1:25,000, which is much lower than the reported worldwide incidence.
The clinical presentations are variable, ranging from mild symptoms, like drowsiness and fatigue, to vomiting, coma, convulsions, rhabdomyolysis, arrhythmias, cardiomyopathy, hepatomegaly, hepatic dysfunction and symptoms similar to those of Reye’s syndrome.
Laboratory findings are: nonketotic hypoglycemia, elevated transaminase and lactic acid levels and hyperammonemia. Metabolic crises are often triggered by periods of fasting, stress, surgery or infection.
It is estimated that approximately 1-3% of unexplained sudden infant deaths are caused by undiagnosed MCAD deficiencies. However, since the incorporation of tandem mass spectrometry into the National Screening Program in 2006, early detection has been easy and fast. In Costa Rica, there have been no unexplained sudden infant deaths related to positive newborn screening results for MCAD deficiency.
To make a definitive diagnosis, the results from the acylcarnitine profile and the urinary organic acid analysis must be interpreted together with the molecular studies. There is the possibility of prenatal testing in the case of high-risk pregnancies, and when the mutation has been identified in one of the parents or siblings.
MCAD deficiencies are inherited in an autosomal recessive pattern; parents who are carriers have a 25% chance of passing the disease to their children. Due to the possibility of having relatives with the enzyme deficiency who are asymptomatic, it is recommended to carry out biochemical and molecular analyses.
Regarding our patient’s treatment, specific medicine for DMD is still not available, but medical intervention seeks to decrease complications and improve quality of life. Corticosteroids have proven beneficial for their strong anti-inflammatory effect, improving strength and prolonging ambulation, reducing progression of scoliosis and delaying decline in respiratory and cardiac function.
Treatments must be determined by evaluating each case individually, taking into consideration the secondary effects of steroids and any pre-existing conditions a person may have. Adverse effects of corticoids exist, commonly resulting from long-term use, and primarily affect children; our patient in particular was affected because he presented with another concomitant disease. Growth, blood sugar levels and blood pressure must be evaluated to successfully control the disease; if side effects are unmanageable, it is necessary to reduce the dosage or change the administration schedule.
Furthermore, in this case, strict nutritional control must be provided to prevent hypoglycemic episodes, specifically by quantifying caloric intake, regulating meal times and avoiding periods of fasting.
As we mentioned, the presence of these two genotypically different conditions in the same individual makes this a very particular and interesting case. According to the review that was completed for this article, no patient has previously been described in medical literature as having both Duchenne muscular dystrophy and MCAD deficiency.
Both conditions can present with the same clinical findings of fatigue, muscle weakness and cardiac injury. However, because they have different medical treatments, finding the necessary balance in the patient’s health becomes quite a medical challenge.

Hay quienes saborean las mieles del fruto; otros saborean las mieles de su producción. En Zarcero, donde recién se realizó la jugosa Feria del chiverre, una de sus mayores comerciantes nos reveló los pormenores de su dulce oficio
El camino a Zarcero se demarca por chiverres a ambos lados de la carretera. El camino avanza, el paisaje se mantiene hermoso, abundan los potreros, pero la atención se desvía invariablemente hacia las cucurbitáceas gigantes colocadas en ambas orillas.
En algunos puntos del trayecto se ven acompañados con un rótulo que indica el precio: (¢1.500 dice el más barato sobre la ruta), pero otros –mudos– parecen recién caídos del cielo, hasta se podría suponer que llegaron con la última lluvia de chiverres. Si así fuera, en la zona de Zarcero hubo un aguacero.
El fin de semana pasado, la Feria del chiverre se realizó con mayor éxito que nunca, según cuenta Elizabeth Rodríguez, quien empleó el contenido de 450 frutos oblongos en los productos que preparó con sus manos y que se le agotaron a las 2 p. m. del domingo, mientras que la jornada terminaba a las 9 p. m.
Ese lujo de venta lo tienen muy pocos, pero en su caso, el mérito también es que ella es la cocinera con la producción más grande para la temporada, según confiesa.
En su puesto, sin logo ni marca propia, no tenía fama más allá de que ahí se vendía mercadería casera, pero, siendo así, ¡qué casa!: 150 cajas de chiverre cristalizado, 150 trenzas, un sinfín de cajetas y al menos 900 paquetes de empanadas, que, en total serían más de 5.400 unidades. Además, este año, la cocinera se atrevió a revoluciar su propia oferta y se aventuró a hacer vino de chiverre. Las 15 botellas se le fueron en el primer día.
Se acabó la feria y, en su casa, en Laguna, está a punto de acabarse el ingrediente estrella del cantón zarcereño. No tiene reparo en aceptar que, por razones obvias, está hastiada de los chiverres. Sin embargo, a petición de este medio se obligó a preparar un poco más de aquel producto cuyas mieles se disfrutan aquí, allá y acullá, especialmente cuando arriban estas fechas religiosas, en una relación inseparable que uno intuiría es bíblica: Semana Santa – empanadas de chiverre.
Habría un par de secretos que no revelaría, nos dijo desde que la abordamos en la amplia cocina donde el horno de leña es el chineado. A nadie le ha revelado nunca la receta de sus cajetas de semillas de chiverre.
"A mí me han dicho que la comparta por si me muero, pero todavía no me estoy muriendo", comenta con la sonrisa inseparable.
Le han dicho que tal vez exagera con la confidencialidad que envuelve aquel platillo, pero a ella no le importa, y prefiere cocinarlo exclusivamente de madrugada, para que nadie vea cómo la hace.
Al chiverre lo trabaja solo para esta temporada. El resto del año, ella y su esposo manejan una soda escolar en la zona.
Geovanny Marín, un hombre de ojos claros y manos gruesas, es su socio número uno en cada empresa implicada esta iniciativa familiar. Ella lo llama y el marido pela un chiverre globoso en cuestión de tres minutos.
Le pasa el machete con fuerza por aquí, con ángulo por allá. Zas, zas, zas. Queda descascarado.
"Las señoras de antes", dice el señor, "lo calentaban en horno para descascararlo pero eso lo quema, se suda por dentro y el vapor impide que el proceso de secado se haga bien". Hay que saber escogerlo. Ahí está el secreto del buen fruto.
Doña Elizabeth explica así: "El pezón tiene que estar bien pegado y seco. Así se sabe que el chiverre está sazón. Si compra un chiverre de ¢1.500 es una cosita pequeña lo que le saca y es una sopa lo que le está comprando".
Después de lavarlo con agua, prosiguen los cortes en tajadas para extraer el relleno. Es triste pero inevitable. Un chiverre de unos seis kilos, dará, a lo mucho dos kilos de relleno utilizable. Uno de 16 kilos, en cambio, dará unos cinco de miel.
Por el trajín de los últimos días, Don Geovanny tiene las manos sensibles, magulladas, irritadas. El chiverre, por más rico que llegue a ser, le abre las manos a cualquiera. No es malo, solo recuerda que la misión para convertirlo en miel, no es para cualquiera.
No hay otra forma de extraerlo que no sea con la mano. Se le quitan las semillas, también las hebras amarillas que no se pueden endulzar.
A pocos, con una buena piedra (o más industrialmente, con una buena máquina), se machuca el chiverre hasta hacerlo hebroso.
"Se sabrá que está listo cuando esté seco", comenta la cocinera, quien además comparte el insospechado uso que puede tener la centrífuga de la lavadora: ayudar a secar el chiverre en cuadritos, metidos en un saco de manta.
Todo aquel contenido, color verde blancuzco, irá a dar a una olla donde será mezclado con tapa de dulce y hojas de higo. Así lo prepara doña Elizabeth, aunque otras personas preferirán condimentarlo con piña, clavos de olor o ralladura de naranja.
El chiverre toma el color del dulce mientras se cocina. En el fuego se suaviza, se suaviza, se suaviza y, cuando han pasado unas cinco horas, tal vez ya está bien suavizado.

Solo por un antojo, por una emergencia, o porque el chiverre no deja de ser delicioso en ninguna temporada, doña Elizabeth y don Geovanny conservarán 25 kilos de miel de chiverre para el resto del año, sin peligro de que se añeje aunque no se refrigere.
Curiosamente, en su casa, a uno de sus hijos no les gusta ese producto, aún a sabiendas de lo bueno que se pierde.

There are those who savor the sweet product, and others who enjoy the sweetness of its production. In Zarcero, a picturesque town 67 kilometers southwest of San José where the juicy Chiverre Fair was recently held, one of the largest, most successful merchants told us details about her sweet business.
The road to Zarcero is lined on both sides with chiverres—white, watermelon-sized squash with pumpkin-like flesh. As the road continues, the landscape remains beautiful, full of pastures, but what really catches the passerby’s eye are the massive squash on both sides of the road.
At some points, the route is marked with signs saying the price: (the cheapest along the way states ¢1.500, about $3), but others–silent¬–seem as if they had recently fallen from heaven, and it could even be assumed that they came with the last chiverre rainstorm. If that were the case, there would have recently been a downpour in the Zarcero area.
Last weekend was the Chiverre Fair (la Feria del chiverre in Spanish) and it was more successful than ever, according to Elizabeth Rodríguez, who used 450 of the oblong squash in her homemade goods that sold out at 2pm on Sunday, despite the event ending at 9pm.
Very few people have the luxury of selling so much. In her case, however, she confesses that she is the cook who produces the most goods throughout the season, which is a success in itself.
At her stand, without her own logo or brand, people recognize her because she has sold her own homemade merchandise there before. Even still, her selection is impressive: 150 boxes of crystalized chiverre, 150 loaves of braided chiverre bread, innumerous cajetas (similar to fudge), and at least 900 bags of empanadas, which, come to a total of more than 5,400 units. Additionally, this year, the cook tried something new and made chiverre wine. The fifteen bottles sold out on the first day.
The fair has finished and, in her house in Laguna, the celebrated ingredient of Zarcero canton is about to run out. There is no solution except, for obvious reasons, being sick of chiverres. However, as per our request, she must prepare a bit more of the product whose sweet jelly is enjoyed both near and far, especially when those religious celebrations come around: due to the inseparable biblical relationship between Easter week and chiverre empanadas.
There are a few secrets that she would not reveal, we were told from the moment we approached the large kitchen where the wood oven is the star. She has never revealed the recipe for her chiverre seed cajetas to anyone.
“They have told me to share it before I die, but am not dying anytime soon,” she told us with a smile.
They have told her that maybe she exaggerates the secrecy surrounding her prized dessert, but she does not care and prefers to prepare it in the middle of the night so that no one sees how she makes it.
She only works with chiverre during this season. The rest of the year, she and her husband run a nearby school cafeteria.
Geovanny Marín, a light-eyed man with large hands, is her number one partner in every business that the family is involved in. She calls her husband and he peels a round chiverre in a matter of three minutes.
He wields his machete with force around here, angled that way. Zas, zas, zas, and it is peeled.
“In the past, women used to heat up the oven to peel it, but that would burn the chiverre,” said the man. “It sweats on the inside and the vapor prevents it from drying out properly.” You have to know how to choose it. That is the secret to a good chiverre.
Doña Elizabeth explains it this way: “The nipple must be dry and well attached to the squash. That is how you know that the chiverre is ripe. If you buy a one for ¢1.500 (just shy of $3), you are getting a small little thing; you are buying enough to yield a soup.”
After washing it well with water, they start cutting it into chunks to remove the insides. It is sad but inevitable. A six-kilo chiverre (roughly 13.2lbs) will yield, at most, two kilos (4.4lbs) of usable flesh. A 16-kilo one (35.2lbs), however, will yield five kilos (just over 11lbs) of sweet jelly.
Because of the hustle and bustle of the last few days, Don Geovanny’s hands are sensitive, bruised and irritated. The chiverre, as delicious as it becomes, will leave anyone’s hands cut up in the process. It is not a bad thing, but it reminds us that not just anyone can take on the task of transforming the squash into jelly.
The only way to remove the insides is with your hands. The seeds are removed, as well as the yellow fibrous strings, which do not contribute to the sweetness of the dish.
In batches, the chiverre is mashed with a good rock (or more industrially, with a good machine), until it becomes stringy.
“You know it is ready when it is dry,” says the cook, who also shares with us an unexpected use for your washer’s spin-cycle: put chopped chiverre in a muslin sack inside the washer to help dry it out.
All of that, greenish-white in color, will go into a pot where it will be mixed with tapa de dulce (raw sugar cane) and fig leaves. That is how doña Elizabeth prepares it, at least. Other people prefer to season it with pineapple, cloves, or orange zest.
Chiverre turns the honey-brown color of the sweet jelly as it cooks. Over the flame it softens, softens, softens, and then, after five hours or so, will maybe be soft enough.
The fair is now over, meaning that it is the best time of the year to get the chiverres that will be used for next year’s production. They must be stored and allowed to dry until 2016.
Be it a craving, an emergency, or because chiverre is delicious all year long, doña Elizabeth and don Geovanny will save 25 kilos of miel de chiverre (sweet chiverre jelly) for the remainder of the year. There is no risk of it spoiling even though it is not refrigerated.
Oddly enough, in their house, one of their children does not like the treat, despite knowing the deliciousness that he is missing out on.

I am a freelance translator from New York, currently based out of Costa Rica. I am pursuing my Masters in Translation (SP-EN) at Universidad Nacional in Costa Rica. Currently, I'm working on my thesis.
I have been living in Costa Rica since 2012 studying and working in translation (both in-house and freelance). I am five courses away from graduating with a Masters in Translation from Universidad Nacional (Heredia, Costa Rica) and worked for a year at an in-house translator for a company called Tiger House Translations. This in-house experience is something that I am truly grateful for as a freelance translator, because it has given me the opportunity to work with a wide variety of texts in all sorts of fields (legal, medical, education, tourism, economics, art, marketing, etc.). It has also allowed me to work alongside other translators in a fast-paced environment, learning from their expertise, working with tight deadlines and complying with high standards for quality. I am proud of my freelance experience as well as I have maintained long-term relationships with all of the agencies that I have provided my services to.
