Source text - English anticancer strategies involving the vasculature
The growth and metastasis of solid tumors critically depends on their ability to develop their own blood supply, a process known as tumor angiogenesis. Over the past decade much work has been performed to understand this process, and modifying this process provides a key point of therapeutic intervention in the fight against cancer. This review explores the development of anti-veGF-based antiangiogenic therapies, of which there are currently three licensed for clinical use worldwide. Although originally anticipated to inhibit the growth of tumor vessels, the induction of vascular normalization caused by these approved agents has provided a novel means of effective delivery of known chemotherapeutic agents. The development of small molecules that target veGF receptors has resulted in the generation of inhibitors with not only vascular activity but anti tumor activity in certain cancers.
This review will address the current status of vascular-disrupting strategies, such as therapies designed to induce tumor collapse by selectively destroying existing tumor vessels. These therapies can be broadly divided into small-molecular-weightvascular-disruptingagentsandligand-directedapproaches. We discuss the current status of development, drug mechanisms of actions, combination with conventional chemotherapy and radiotherapy, and potential future targets for therapeutic intervention.
Tumor vessels are structurally abnormal; they are leaky, tortuous, deficient in pericyte coverage, and lack the normal hierarchical arrangement of arterioles, capillaries and venules.Consequently, tumors tend to be poorly perfused and also have a high interstitial pres-sure because of a lack of lymphatics. Both of these factors compromise the delivery of chemo therapeutics. This poor perfusion results in a highly hypoxic and acidic tumor microenvironment, and this combination of features is known to affect the physiology and gene expression of tumor endothelial cells. Tumors can use a variety of strategies to enhance their blood supply, including sprouting angiogenesis, vessel co-option, intussusception of existing vessels, and the recruitment of bone-marrow-derived endothelial progenitor cells into growing vessels. Vessel co-option involves the tumor, particularly in the case of metastases, growing in proximity to the pre-existing vasculature and deriving nutrients from it. Vessel intussusception is a process whereby vessels split longitudinally and thereby increase vascular density. Unlike sprouting angiogenesis, this process is fast and provides a rapid means of increasing vascular density without requiring the proliferation of endothelial cells. By contrast, sprouting angiogenesis is a slower, invasive process that enables the formation of vessels from pre-existing vessels, which are able to bridge a vascular area. Sprouting endothelial cells produce proteases to degrade extracellular matrix proteins; the cells subsequently migrate, proliferate and join other existing vessels. Finally, the endothelial cells form a lumen and recruit pericytes to stabilize the newly formed vessel.4the process of tumor angiogenesis is promoted by proangiogenic factors such as veGF, fibroblast growth factor (FGF) and interleukin 8, which are produced by tumor cells and cells of the tumor stroma, with the most critical being veGF. The veGF family of growth factors comprises veGF a–e and placental growth factors (PlGF) 1 and 2. VeGF a is the predominant mediator of angiogenesis in tumors.8veGFinducesangiogenesisvia binding and activation of the tyrosine kinase veGF receptors 1–3 (veGFr1–3). VeGF a can bind and activate bothveGFr1 and veGFr2, but signaling through veGFr2 ismorepotentthansignalingthroughveGFr1atinducing an antigenic response in endothelial cells.9upregulationof veGF production can be caused by hypoxia,8andvia growth factors such as epidermal growth factor (eGF), and platelet-derived growth factor (PDGF).Data from animal models in the field of antiangiogenic agents has not always translated to the clinic. Despite this, blockade of veGF signaling in a number of different experimental xeno graft models was shown to reduce tumor growth from 25–95% in human xenograft models using different tumor cell types grafted either orthotopically orectopically,and these results paved the way for the clinical development of veGF signaling antagonists.
Bevacizumab the first antiangiogenic drug to be licensed in 2004 by the FDA was bevacizumab (a humanized anti-veGF antibody)—as a first-line treatment for metastatic colo-rectal cancer in combination with 5- fluorouracil (5-Fu)-based chemotherapy. Subsequently in 2006, it was also licensed for second-line treatment of colorectal cancer in combination with 5-Fu, and also in the treatment of competing interests.

يعتمد نمو الأورام الصلبة و تشكيلها النقائل بشكل أساسي على قدرتها على تطوير توعيتها الدموية، و تعرف هذه العملية بالتكون الوعائي للورم.
و قد أجريت العديد من الأبحاث خلال العقود الماضية لفهم هذه العملية و العمل على تعديل هذه العملية حيث أن هذا يشكل النقطة الرئيسية في التدخل العلاجي لمكافحة السرطان.
و هذه الدراسة توضح تطوير أضداد علاجية مضادة للتنشأة الوعائية للورم، و هي عبارة عن ثلاثة طرق مرخصة للتطبيق السريري في جميع أنحاء العالم.
و مع أنه من المتوقع أصلاً أن تمنع هذه الأضداد نمو الأوعية الدموية ، إلا أن هذه العوامل هي التي تقوم بالتحريض الطبيعي للأوعية و هذا ما قادنا إلى طريقة العلاج الفعالة المعروفة بالمعالجة الكيماوية.
إن تطوير الجزيئات الصغيرة التي تستهدف مستقبلات عامل نمو بطانة الأوعية veGF قادنا إلى جيل من مثبطات للنشاط ليس لمكافحة النشاط الوعائي فحسب و إنما لمكافحة نشاط كامل الورم في أورام سرطانية معينة.
ستستعرض هذه الدراسة التقنيات المستخدمة حالياُ في تعطيل الأوعية الدموية، مثل العلاجات المعتمدة على العمل على القضاء على الورم من خلال القضاء على الأوعية الدموية للورم.
فنحن الان بصدد مناقشة كل من تطور والية تأثير الدواء ، مشاركة العلاج الكيماوي مع العلاج الاشعاعي والأهداف المستقبلية المحتملة للمداخلة العلاجية.
تكون الأوعية الدموية للأورام شاذة بنيوياً حيث تتميز بأنها ذات نفوذية ، ملتوية ، تعاني من نقص في الطبقة المحيطية و عشوائية أي ليست ذات ترتيب هرمي طبيعي للشرايين و الأوردة الصغيرة و الشعيرات.

تقوم الأورام بتطوير توعيتها الدموية بعدة أساليب تتضمن: التنشأة الوعائية بالتبرعم ، المعاوضة الوعائية ، انغلاف الأوعية الدموية الموجودة ، استقطاب الخلايا الدموية ما قبل الأم المشتقة من العظام الى الاوعية الدموية الناشئة ، لاسيما في حالة النقائل حيث تنمو بالقرب من الأوعية الدموية الموجودة و تستمد تغذيتها منها .
انغلاف الأوعية الدموية عملية يتم بموجبها انقسام الأوعية طولياً و بالتالي زيادة كثافة التوعية الدموية و خلافا للتكون الوعائي بالبرعمة تكون هذه العملية سريعة تشكل اسلوبا سريعا لزيادة كثافة التوعية الدموية بدون الحاجة الى انتشار الخلايا البطانية ، بينما يكون التنشؤالوعائي عن طريق البرعمة على نقيض ذلك أبطأ و عبارة عن عملية اجتياحية تعتمد على تشكيل الأوعية من الأوعية الموجودة مسبقاً و التي تكون قادرة على تشكيل جسور في المنطقة الوعائية ، تنتج الخلايا البطانية المتبرعمة مادة البروتياز بهدف تحليل القالب البروتيني خارج الخلوي ثم تهاجر الخلايا في وقت لاحق و تتكاثر و تنضم للأوعية الدموية الموجودة مسبقاً ,و أخيراً تشكل الخلايا البطانية تجويف و ترتصف لتحقق بذلك استقرارالأوعية الدموية الجديدة.
حيث تكون عملية تكوين الأوعية الدموية الورمية مدعومة بعوامل مثل veGF و عامل نمو الخلايا الليفية و الانترلوكين 8 و التي تفرز من الخلايا السرطانية و خلايا النسيج الضام للورم .
وتضم أسرة العوامل المساعدة على نمو الأوعية كل من veGF العوامل المساعدة على نمو الأوعية veGF من a و حتى e و عوامل النمو المشيمية PlGF) 1 و 2
غالبا ما يلعب ال VeGF دور الوسيط في تكوين الأوعية الدموية للورم ,بينما يقوم8veG بربط و تنشيط مستقبلات التيروكيناز لعامل VeGF 1–3 veGFr1
و يقوم عامل VeGF بربط و تنشيط كل من veGFr1 و veGFr2 ولكن التحريض من قبل veGFr2 و الذي يكون أقوى من التحريض من veGFr1 يؤدي الى الاستجابة المستضدية في بطانة الأوعية الدموية.
و تتم زيادة كمية انتاج عنصر ال veGF تتم بسبب نقص الاكسجين و عوامل النمو مثل عامل النمو الظهاري (eGF) و الصفيحات المشتقة من عامل النمو (PDGF).
وعلى الرغم من عدم نقل النتائج المستخلصة من التجارب على الحيوانات في مجال العوامل الجينية المضادة للأوعية الى التطبيق السريري ، إلا أن العديد من التجارب القائمة على حصر التحريضات الصادرة من العامل veGF في تجارب الطعوم المتخالفة على الانسان أظهرت تراجعاً في نسبة الورم بمقدار 25 - 95 % عند استخدام أنواع مختلفة من الخلايا الورمية إما عظمية داخلية المنشأ أو خارجية المنشأ.
و قد مهدت هذه النتائج الطريق للتطور السريري لمضادات المحرضات لعامل veGF .
Bevacizumab هو أول دواء يكافح النمو الوعائي جينياً تم ترخيصه من قبل منظمة الصحة و الدواء FDA ( الأضداد المضادة للعامل veGF )و يعتبر الخط الأول في العلاج في سرطان القولون و المستقيم ذو النقائل بالمشاركة مع المعالجة الكيماوية بالفلوريوراسيل-5
ومن ثم تم بعد ذلك لاحقاً عام 2006 ترخيص الخط الثاني في علاج سرطان القولون و المستقيم بالمشاركة مع الفلوريوراسيل -5 .
