My doubt is, can you refer to something as 100% vegetable when referring to fruit? Or should it be 100% fruit/plant? (for example).

"As an adjective, the word vegetable is used in scientific and technical contexts with a different and much broader meaning, namely of "related to plants" in general, edible or not — as in vegetable matter, vegetable kingdom, vegetable origin, etc."

However if the extract is actually a dietary supplement, then you could of course use "herbal".

I agree with Taña. If this is a juice, it would be 100% fruit juice--that is what I always see on bottles. I'd need more context to venture this answer, though. Is it a juice?

Asker: No Joshua, it is not a juice, it is an extract, that is the ONLY reason I questioned the term as it is related to polyphenols, antioxidants, etc.
