eje CC = eje cabeza-cola

Explanation:
Based on the measurement of 13.7cm, this is about pancreatic length. The craniocaudal dimension of the pancreas (with "cranial" and "caudal" referring to the axes of the patient's body) would be much smaller.

I have never seen any reference to a measurement "along the head-tail axis" of the pancreas, hence, I would suggest to keep it simple and just use "length". (On the other hand "eje CC" does not seem to be all that common in Spanish, so why not use an uncommon term like "head-tail axis" in English as well?)

agree  Chema Nieto Castañón: It is the spleen and not the pancreas, and yet I think your analysis is correct; long/longitudinal axis looks like the most natural option here. ¡Saludos!
